Being an IMG, what are the toughest things in getting a residency in the US.?

If you ask a silly question, you get a silly answer...
For a US-IMG with perfect English and Greencard, it may be USMLE.
For an Asian with a heavy accent who was born outside, it may be ENGLISH or a visa to come to Pensylvania for CSA test!!!
