Easy as pie . Just send them a brief but polite letter stating that you would like to withdraw your application. No need to go into great detail.

Yep, SouthernGirl's right. No official forms, just your decision to withdraw in writing.

yep, just write and tell them. To be honest when it's been pre-interview I have just done it by email but for some reason post-having done one of those 'intent to matriculate' I would feel it necessary to send something in paper/ink by regular mail.
