Brief summary of myself:

20yo caucasian male (US/Canada dual citizen)
3.85 gpa (had a 3.4 my first semester but have been 4.0 for two years now)
518 MCAT
Biology BS, Chemistry and Art History Minors

2+ years doing medical research (asthma research lab). I have some presentations and posters but nothing beyond the state level. I got a semi-prestigious research fellowship from my school last summer and this summer I have an internship at the NIH. 2nd author on a paper in a upper/mid tier journal (I think AJP qualifies as such).

For clinical experience, I volunteer at a hospice over holiday break (~75 hours total so far), had a diagnostic medicine internship at a regional medical center (200 hours) and have shadowed a mix of different physicians (50 hours)

As far as nonclinical activities, I'm a big brother big sister community mentor, I have been assisting in a language teaching program ESL refugees for 1.5 years (had to take this semester off but will resume), was a reading buddy at a school in a low ses area for 1 year, and have been a teaching assistant for the chemistry department at my school. I've been a member of a pre-med club for 2 years (doesn't mean much I think). Also am an Eagle Scout.

My school list right now is as follows:
UNC (in-state)
Eastern Carolina
Wake Forest
Duke
Vanderbilt
Emory
University of Virginia
VCU
ETSU
Dartmouth
Georgetown
Temple
George Washington
Tufts

I know I need a bigger list and am not really sure where to go from what I have right now. Also I am concerned that my lack of leadership will keep me from many schools on my list. Would appreciate any input I can receive, thanks!

You have a good list of schools already. You could add Einstein, Mount Sinai, Hofstra, Drexel, Temple, Jefferson.

ETSU takes very, very few OOS applicants so it's probably not worth applying

I just want to say that your stats/EC look really good. Super jealous right now! But you deserve to be interviewed and accepted in the schools you have chosen. Good luck on your cycle!
