So then how should the nurses be billing when pretty solo in OB or in GI or other remote locations where the doc is supervising from far away.
 
Unfortunately in most cases it's not the CRNA that is using QZ modifier, it is the CRNA's employer, because QZ allows for maximum billing without having to satisfy the requirements of medical direction (present for induction, present for emergence, present for key points, and immediately available), and allows flexibility in the number of rooms an anesthesiologist can supervise since there is no billing under the anesthesiologist's name.

After rotating in the sicu and micu, I don't understand how they tout this as "experience."

Every dumb question I get about an icu nurse wanting to do something they think is helpful actually isn't even remotely helpful and any difficult decision, the first thing out of their pie holes is, "I'm just the nurse, what do you want to do Doctor?"

Yes, every freaking crna touts their ccu experience as "elite training." It's complete bs! Basically inflating their ego so they can be even more dangerous giving anesthesia.

An example from today: "dr j, I want to order a BMP on my patient." Yes, not can I or should I? straight this is what I want. Reason: "hes peeing a lot (50-70cc/hr) and I think since he is, his potassium is low."

On lasix...... No
Any renal issues.... No


End rant ! I was going to make a point but it got lost in my rant over nursing and their "icu experience."
 
Point I was making is crna's tout their equivalence in part due to their "experience in the icu." somehow taking orders in the icu versus on the floor makes them more superior than regular nurses and that with a little extra training they're equivalent to physicians.

My rant was giving an example that majority of the time this experience isn't really true decision making/training, but I'm preaching to the choir.

I vomited in my mouth like 12 times while listening to this guy drone on and on....including every time the hosts referred to him as "doctor". Gross.
I can just hear it now...."yes Sean and Rich, I will agree to an interview but remember when you address me it's DOCTOR Gary Bridges. Got that? DOCTOR."
