How do you distinguish between them?

ADE: multifocal periventricular inflam & demyelination post measles, VZV, or smallpox/rabies vaccine

SSPE: no Ab to M component => failure of clearance => inflammatory/demyelination/gliosis years later.

Is the only difference the timeline (and that SSPE only occurs after measles)?

SSPE will be an immigrant (not vaccinated) who exhibits poor school performance and behavioural change in late adolescence usually. If there's no immigrant status, the USMLE will make it clear there was an infection with measles rash early in life and then tell you there's behavioural change and poor school performance.

I know I didn't fully answer your question, but ive only ever seen SSPE questions.
