I am an IMG and applied for anesthesia residency this year. Now I am thinking of pulling out of match and intend for the next one. On the other hand, to wait for another year is a big decision (or gamble). Here is my background: *graduated from the best medical school in my country (not intend to self-promote, my school is known in US's premier programs), GPA 4.6/5
* Ph.D in neuroscience in one of the top three medical schools here. Research concerns ion channels in the central nervous system which is highly related to anesthesia. PI National Academy member
* Step 1 and 2 CK around 240

My problem is I failed CS for the first time. It was my bad and I only got it cleared at the end of last month. By then, the interview season was largely done. I applied in late Sept. and got some interviews before my CS report on Oct. 10. After that, I did not get any. Unlike US graduates, we need to clear our CS and get ECFMG certified before joining the match. Apparently anesthesia is very competitive and the programs have tons of applicants.

My tentative plan is going to academics in the future. The programs I interviewed are mostly middle-tier ones and not strong in research. Admittedly, to some extent I still have the mental knot of reputation. I know in the end this kind of mental obsession is nonsense; but I am in NOW.

So the question is : should I pull out or not?

I would scramble into a surgery prelim year and then try to pick up a CA1 position outside the match next year. At least you will be on your way to becoming licensed in the U.S.

I agree with the other poster, take a surgery prelim at a program that reserves anesthesiology CA1 spots and where you would like to go. Great advice.

When I checked the websites of the programs, very few will state if they have out-of-match positions though.
 
I would try to go to a big city that has a lot of programs. There are usually openings that go unadvertised. You could also go for prelim medicine. If you don't get a CA1 spot you could do two more years and at least have a license to practice.
