can an direct inguinal hernia push into the scrotum

For direct hernia - it just means that it is coming through the body wall (area of weakening in the body wall). However, once it goes through this weakness, it can come into the inguinal canal and therefore can go into the scrotum - although this is rare. Indirect hernia comes in through the deep ring and thus always comes into the inguinal canal and then usually into the scrotum.

I think that is correct, but maybe someone can second that!
 
if it goes through the anterior wall, wouldn't it be anterior to the inguinal canal? and if it was, wouldn't the transverse abdominus and the fascia who's name i cannot remember seal it out of the canal?

I do second that. I am a first year who normally wouldn't know very much but we just covered this. It is very rare, but a direct hernia can enter the scrotum.
 
I third that. Indirect goes through the inguinal canal and can end up pushing into the scrotum. Direct simply pushes through the abdominal wall, and for practical purposes, does not go into the inguinal canal or scrotum.
 
indirect is more likely to go into the scrotum because of the tunica vaginalis (that layer of fascia that follows the nuts as they descend)....this is an inherent weakness in the wall at that point, sometimes is still patent....abdominal contents can follow the t.v. down into the scrotum. That's why indirects, but usually not directs, go into the scrot.
 
I've seen a case where it's gone all the way into the scrotum. Patient was about 30yrs old. This ws in a rural hospital in Africa where I was shadowing some docs. When we first saw it, we were all like 😱 Looked like the patient had a huge mass of ~ 7cm in diameter hanging between his legs. It's only after the docs cut throught the scrotum, that they found that a good portion of his intestines had descended into the scrotum, and it was strangulated and essentially dead, so they had to do a bowel resection. And that's when they found out he had undescended testes as well.

So yes, it can happen. Though in the US I think you're more likely to see it in kids, but not to this extent.
