claritin vs. prilosec: why wait at lest. 2 hrs apart?

1. pt taking both, calls in & asks RPh how to take them.

3. my stance: that answer isn't evidence based, b'cause neither clinical trials nor reliable tert. lits, eg micromedex, eFacts, ever state a conclusion like that.

Omeprazole should be taken at least 30 mins before food because food increases the stomach pH and therefore, this decreases the absorption of omeprazole.

Loratadine is a H1-receptor antagonist and therefore, it also increases the stomach pH. This, in theory, explains why loratadine and omeprazole should not be taken together. However, I didn't find any published article in pubmed written about this topic.

Theoretical drug interactions are not drug interactions. I found a couple of papers discussing a potential kinetic interaction regarding first pass intestinal metabolism and the p-glycoprotein pathway; however, it was looking at fexofenadine and omeprazole (both agents are substrates). Once again, nothing of any statistical or clinical significance was found.

Did you ask this pharmacist what they were basing this recommendation on? Sounds like your stance is correct, perhaps someone else will offer something I am not thinking of. But probably not.
 
Realistically, at steady state - it doesn't matter.
