Degree Translation

Can anyone tell me how I can get my degree translated into English? I have my MD, PhD from Poland and wanted to have this in English. Also, my last name changed since medical school because I am now married. Is there a way for this to reflect on my degree?

My Residency and fellowship "degrees" are of course in English with my proper married last name.


Any help would be great, as I am heading to my country in a couple week and would like to take care of this ASAP.
