Fischer projections HELP!!

When you read long Fischer projections do the horizontal lines always indicate that they are coming out towards you or do the horizontal lines alternate between coming out and going in?

Because if the horizontal lines always indicate coming out of the page, then wouldn't the molecule wrap and touch itself?
