Utah Med is the mouthpiece of the devil. Don't let them sway you towards the darkside! They must be confusing the
clinical test of CN IV with the
action of the superior oblique muscle. By gazing medially, the axis of the eye is aligned with the muscle, thus better isolating the superior oblique for downward gaze.

And yes, I agree that we should remove the blasted thing at birth from now on. However--that presents us with a problem: which way will the eye deviate without the function of CN IV and the superior oblique?!?
