I asked a professor today for a LOR which she agreed to do, but asked that I put together a portfolio with work experience, volunteer activities, shadowing experience, and my personal statement.

Have any of you done this? I'm assuming it's similar to a CV but I wasn't sure what type of format I need to use. Is it necessary to put my major and school attended on the form?

Yes thats a pretty normal procedure, at least at my school. The professors always want your up to date resume/transcripts/statements (aspirations) and along with those they usually set up a personal meeting before they write the letter. I would include everything you would like addressed in the LOR.

I just gave them a folder with my transcript as well as a letter that laid out my background, course work/extracurriculars while at school, plans for the future, etc. I also included a thank you letter and directions for them to submit the letter of recommendation (I opted to not have access to the letters and they were submitted straight to the premedicine office at my school which subsequently mailed them out to the schools I applied to). I didn't write anything like a resume or CV. Personally I think a letter is better than a resume because it allows you to go into a little more depth and they have more substance to formulate their own letter from.

I gave them a resume, personal statement, and transcripts if they requested them.
