If I was to shadow a hospitalist who specialized in Internal Medicine, would medical schools consider that as primary care exposure or not? I understand that the nature of the work is somewhat different, but the training to do both is the same.

Sorry, not the same thing. You'll need to do more shadowing to get the credit.
 
Not exactly the same...related, but not the same. Also, keep in mind that many med school admissions committees tend to be disproportionately made up of primary care physicians and administrators who want to promote primary care. Just something to keep in mind...
