How common (or uncommon) is it for a professor to forget to write a LOE?

Na. They are really busy folks with many others asking them for the same thing. It's fine to send them a reminder a week or so before.
 
No it's normal. I've 5 letters. I've had to remind all 5 writers to write or submit the letter.

One i've even reminded three times.

OH CRAP i have to remind him again today!

They are busy people. I had to remind them a bunch of times to get them to get my letter written and ready to go. It was pretty annoying to deal with honestly.
 
Not a bad sign, pretty common in fact. For one letter, I had to wait over a year. For another, about 8 months. They have higher priority things going on and your letter finds its way to the bottom of the pile. Keep reminding them (politely!).
 
Fairly common, but several "I forgot"s means that you should look for someone else.
