Yes network adequacy is the law. Otherwise the insurance companies lowball the contracts forcing doctors to be the “out of network” bad guys.
 
from what i've heard, the amount of fine is chump change in the big picture.

I never understand why a same service should be paid differently based on insurance or not. Restaurant did not discount my favorite fish dish when I was a poor student.

my favorite is when the insurers talk about how they are going to help cut healthcare costs by decreasing physician reimbursement across the board. Yeah, that's not to help the customer, that's to pad their bottom line.
