Hey everyone. I'm beginning the med school application process, and I've been reading this forum for the past month or so and found it very helpful. Right now I'm a bit confused about exactly what evaluation forms I need to provide for the individuals that will be writing letters of recommendation on my behalf. The pre-health advising office at my undergraduate university has their own evaluation form, which I'm sure I must have each letter writer fill out, but do I also need to provide the letter writers with an evaluation form for each specific school that I'm applying to. In other words, does each med school have their own individual evaluation form that they need letter writers to fill out.
