Macrodantin-BID?

me: You wrote for Macrodantin 100mg BID, I wanted to clarify whether you wanted Macro-BID 100mg, or Macrodantin 100mg QID
Dr: /snide chuckle/ I want Macrodantin 100mg BID
me: Macrodantin is dosed QID for UTI, Macro-BID is BID.
Dr: I want Macrodantin 100mg BID
me: Is there a reason the dosing is unusual? I would really like to learn from your experience.
Dr: /deep sigh/ The dosing is normal
me: Macrodantin isn't long acting or indicated for twice a day dosing...Macro-BID is...hence BID in the name. It needs to be QID in order to reach a bacteriocidal concentration
Dr: I want Macrodantin 100mg BID

I logged the conversation and filled as prescribed.

The question is am I missing something here?

Was it a pediatric patient? Macrodantin q12 can be used for UTI prophylaxis.

I've also seen it used that way in adults, but not as frequently.

An excellent point, Macrodantin is used QID and Macrobid is BID. However, Macrodantin can also be filled BID dosing for prophylaxis.

PS, the snide chuckle/sighing is not needed. We understand some physicians can be arrogant. However, just try to relate to them. Assuming its primary care; OB/GYN, Uro, etc, they are busy people and might not want to explain it to you. They dont get paid for it.

I have had this happen before as well. It was not for prophylaxis it was for treatment. Document the call and move on.

I've never seen Macrodantin given BID. The instances where I called them to change it to MacroBID, they changed it.

Call the patient and see if it worked or not. If it worked you got nothing to say. If it didn't work, tell the patient to sue the doctor.
