The best way to prepare for an interview is to ask questions. Ask current residents or the director what the writing sample entails if they can or how you should prepare. They might send back you won't need any preparation which should tell you is it a very generic writing topic. Or they might provide specific direction. Check your on site invitation again before sending anything just to be sure they didn't provide any direction.
Off recent memory, I interviewed at several MCOs for residency programs. Only one had a writing sample on site. The topic was very broad (write what this topic means to you type of deal) and designed to assess your ability to write a succinct response within a time limit.
