Mixed MD/DO list, but leaning DO. What are my chances?

This seems pretty reasonable. But the pharmacist, dentist, and optometrist hours you list should not go on your application. Those are not physicians. You should probably shadow more physicians in specialties other than radiology.

You probably need a DO letter for some of the schools on that DO school list.

You will need at least a C (not C-) in all prereqs (including physics). Hopefully you retook physics 1.
 
On the DO side Ohio U., Mich. St. & Rowan are heavily weighted towards in-state applicants (85-90% range). They do accept OOS but stats have to be higher than IS. Also agree with comment above regarding eliminating dentist, optometrist and pharmacist from your app. Good Luck!!!
