I know you are coming from a place of pride. Perhaps you are not aware of how strong NYU home programs are currently considered. For lack of a better resource, doximity reputation rankings this year has NYU ranked above Stanford in derm, emergency med, internal med, neurosurgery, neurology, ob gyn, ortho, plastics, psychiatry, radiology, surgery, and urology. In some cases it isn’t even close. Further, I’d offer the argument that NYU students are proven not to be lured by a “name.”


I know NYU derm, plastics, NS, GS, radiology, EM, PMR have been top programs for a long time. I’m sure other departments have climbed in recent years too.

I'm amazed that places like NYC, SF, Boston, and Seattle remain this popular for residency. Due to admissions inflation, med students are much older than they used to be. When you're 30+, you're just too old for that ish, and you've probably got a partner making some sort of income. I can't imagine wanting to keep renting or potentially even having roommates (NYC/SF) at this stage and in this economy.

In other cities like New Haven, Atlanta, Philly, Chicago, Baltimore, Durham, Nashville, Houston, etc... you could buy a very solid place for $30-40K down or at least rent somewhere that doesn't require you to schlep yourself to a laundromat after a 12 hour day. Not to mention having a car, parking, extra cash for conveniences. Having lived in NYC and Boston, life is just hard in these cities with low income, no car, and minimal free time.


It goes the other way too. A lot of people have significant others who have careers too. And career opportunities outside of medicine are better in the large metro areas. If your spouse is a scientist in biotech, you’re more likely to want to be in Boston, SFO, or San diego than Atlanta, Hanover,NH, or Nashville. If your spouse is an attorney who graduated from a top law school, you’re likely to end up in Boston, NYC, Chicago, LA or SFO.

Jobs galore starting at 400-500k even in desirable metro areas. Even more if you’re willing to go off the beaten path.
What is your perspective on if it is getting more competitive though? Everyone is saying it is, which I'm inclined to believe. But app numbers are relatively the same as last year. ~3% increase from US MDs vs a 15% and 17% increase in ortho and rads.

Also, when you say off the beaten path are you referring to rural? I'm applying this upcoming cycle, interested in anesthesia and like rural-ish settings, but heard because of "rural pass through" it was harder to get a job as an MD in a rural area. If I could make more to live where I want to, I'd be in actual heaven. But was kind of accepting I'd, unfortunately, have to live in a more metro area if I did anesthesia.

Anesthesia is a large specialty with many (~2000/yr) training spots. Even if it is getting more competitive, if you are an average medical student from an average medical school and you apply widely, you should have no problem matching. Maybe not your preferred program or preferred location, but you will match.

Well saying it's T20 level is way too far, but it's still definitely impressive

I may be biased but based on my personal experience + the final list, I think Miami can set you up very nicely for ending up at strong programs pretty much anywhere in the country. It may not be as ivy heavy as some top schools but not everyone wants that anyway. As others have said, personal preference plays a huge role in these lists.

I do know there were some people who didn’t match of course (the number I’ve heard is 96% match, not sure if pre or post soap) though several of the ones I know of were in competitive specialties like surgical subs that were bloodbaths across the board

Would be interested in hearing what locales you're talking about that have the same punchiness as the big NYC/CA residencies. I think of Pitt.
Depends on the field, but Penn and Hopkins can run with any institution out there. I'd also put Vanderbilt, Pitt, Baylor, Duke, Michigan, Northwestern, U Chicago, Yale, Emory, and UTSW as functionally equivalent to Cornell, NYU, Sinai, BI, and sometimes Columbia (depending on the field for all, obviously).
Could be that a somewhat high proportion of med students have family money. Not really absurd to think that a resident with a 5000000 trust fund can live above his or her means in the big cities mentioned
Could be. Med students are definitely wealthier than average. I've seen plenty of friends with $300k+ in loans run for NYC/SF though, even without a prestige residency attached to it.
hmmm this might be missing people or have errors? at least it's missing one of my future co residents lol

Tons of people have family ties to the NYC area, or are more comfortable in big cities/want easy access to the things they offer. If your commute/life is easy but it takes you hours to get to a major city, you're never gonna get to do city things on your limited time off. If you really like doing big city things, you want to be living in it already so you can enjoy it in your rare time off.
Kinda disagree with this, but it depends on your tastes. With a $60K salary, I'd argue that Philly/Atlanta/Chicago offer basically everything NYC does at a lower price point, allowing you to do more. You're not doing Broadway very often on a resident's salary. You're right, though. For some people NYC offers some tangible advantages that no amount of money could buy in another city.
It goes the other way too. A lot of people have significant others who have careers too. And career opportunities outside of medicine are better in the large metro areas. If your spouse is a scientist in biotech, you’re more likely to want to be in Boston, SFO, or San diego than Atlanta, Hanover,NH, or Nashville. If your spouse is an attorney who graduated from a top law school, you’re likely to end up in Boston, NYC, Chicago, LA or SFO.
Now that I think of it, if things work out with my SO I'd be in exactly this situation, probably trying to match in Boston/SD/Seattle for biotech opportunities.

They certainly filled far fewer programs this year so there’s no doubt med student interest overall is down, but we should also ask ourselves: was it weird that interest was as high as it was for as long? A significant chunk of classes were all flocking into EM for like 2-3 years as if it were IM (as in a specialty with a ton of positions).

Em will probably get something like a diminished version of what’s happening in rad Onc: ppl who are attracted to the career for all the good reasons the career is attractive for them will still pursue it, but will likely be more selective in where they apply to ensure they have the best chance at having good job opps

UM is definitely comparable to T20s in terms of their match list. There was a graph last year that compared UM's match list to other top schools and UM outperformed a couple of them. When I find it again, I'll link it to this post.

Agree. The quality is really what sets apart a t20 from the rest of the pack. If you look at IM’s matches, Miami really can’t compare to a t20 yet.

Someone brought up how in a few of these lists, Anesthesiology may be up. I wonder if some of the would be EM folks are going Anesth instead.

We can agree to disagree. UM trumps some of the lower Top 20s, not saying it's at Top 10-15 level just yet. Going off DOPEN, I let the results speak for themselves. Either way, the fact that UM consistently has the best match list in Florida and still is ranked lower than UF by USNWR at this point is laughable - funding isn't everything.

By that logic do you believe Case western is better than most of the top 10 because it matched DOPEN the most?

Why the Criteria is only DOPEN ?

Interventional radiology, CT, Thoracic, Urology, Top GS and T10 IM residency programs are equally competitive as DOPEN.

For example, you have to read between the lines— how many are doing Medical or Surgery Prelim years. Or doing an “ocular oncology research fellowship”. You have to assume that most of these people didn’t match into their competitive field of choice.

I feel like the takeaway shouldn't be "Miami is a T20 equivalent" but "Oh wow you don't need to go to a T20 to match into something competitive".

Could this "DOPEN" :rolleyes: conversation be had elsewhere? Not to try to play dictator but could we just avoid it in this thread especially because its being driven primarily by premeds and preclinical students. You all, no offence, just are operating from a poorly informed vantage point. You have to go through some rotations to understand why, for example, the N in DOPEN can be really really undesirable for many people. While others could not be caught dead in the "D" clinic.

Edited to add: The conversation matters, but it's clogging up the thread. And its not interesting.

Wouldn’t be surprised! There’s a lot of overlap in the pros and cons of both like lack of continuity, shift work, lots of procedures, similar income levels, variety of pathology, etc.

Yes. I think we all know that things roughly break down between T5(ish), T20, T100, and then unranked MD vs. DO vs. IMG, but most people aren't wondering whether Harvard or Tufts is going to provide a bigger advantage in the match. Most people are trying to figure out if they want to go to a school ranked 25 vs. 50, or 90 vs. unranked.

What you really need to look for is what the school emphasizes and how it supports students who fail. Figuring this out is a lot harder than running through a match list and counting DOPEN matches or T10 IM spots. My school tends to match relatively poorly compared to similarly ranked schools, and having worked with faculty on the curriculum it's obvious why. They don't emphasize research (they actually shuttle students away from it in favor of things like nursing experience programs or non-clinical community service). They have strict cutoffs for Honors, despite being a high enough ranked school to get away with lenient standards. They emphasize a lot of soft skills that are useful, but hard to track or quantify. They specifically teach away from Step 1. Most importantly, they don't have any real system in place for students who fail to match. Some schools have leadership that is keyed in to how the game is played. Some schools have leadership whose guidance is the functional equivalent of your boomer parents telling you to look in the classifieds for a job.

Outside of the rough framework I listed above, I don't think PDs really care about your school. What matters is whether or not that school sets you up for success. That's next to impossible to read from a match list.

Not better than, but comparable to. I wouldn't go as far as the top 10, but closer to the top 20 is fine. At any rate, Case Western is just outside the top 20, whereas Miller isn't anywhere near it - which makes its match list all the more impressive. Go big or go home, as they say. It's better to end the discussion now, as this thread is getting derailed. I'll let the match list speak for itself ;)
