This may be slightly off-topic, but for the IM applicants here, what type of pros/cons does a career in Allergy/Immunology offer vs a hospitalist or outpatient IM clinic job? I only got brief exposure to a private practice A/I clinic so I am just genuinely curious about your reasons for going into A/I from Internal Medicine.

No field is perfect but A/I comes pretty close. Job satisfaction, compensation, pathophysiology, and patient population are the biggest pros.

It is one of the very few outpatient fields where you get to see results make a difference in a patient's life. You can treat uncontrolled asthma and see/hear them breathing better. You can see the skin changes from their eczema after treatment. You can see/hear the rhinitis symptoms get better. Patient's actually give a damn and are very compliant with the treatment plan (for the most part) because they can see the results too. Versus outpatient IM clinic where if you control hypertension the only kudos is "you didn't have a stroke this year, nice job." Hospitalist work to me felt like being a glorified case manager. I honestly think hospitalist medicine is way more fun when you actually get to do it, but the majority of my time felt like tracking down case managers, specialists, and coordinating when procedures needed to be done, etc. Pages and admits during a busy day are not fun as well.

When you compare the two, compensation is about the same if you look only at the numbers. But if you look at the numbers with context of all the pros that come with A/I, I personally think it's better than hospitalist or outpatient since you don't have to deal with the BS that comes with those settings.

Your patient population expands to peds/adolescent patients as well so you'll get to see a bunch of disease states that you would have never seen before. There's a lot of pathways and pathophysiology that you'll get to re-learn from your days as a med student, but it's way more fun when you understand and actually see the rare cases that you learn about. The patient population is generally healthier than the patients you see in the hospital or outpatient clinic so it's not as stressful. Majority of the time you'll only be sending people to the ED if they went into anaphylaxis and got their epi pen.

When comparing cons to outpatient/inpatient IM, there aren't really any cons in my opinion. Any cons that I can think of in A/I are the same ones you'll deal with in IM. I could go on for hours about why A/I is the best field in medicine lol. Good luck to those applying, if you don't get in this year I encourage you to try again. I can't speak for the peds side, but A/I training is way more fun than IM training. You get to feel like a doctor that your patients actually appreciate.

Anyone heard back from Nemour's/Thomas Jefferson, Brown, or Zucker/Cohen Children's yet for peds?
 
if there are ~200 applicants in the pool, that would suggest they are interviewing nearly 1/4 of all applicants for 2 spots. That is also assuming that everyone is applying to every program...

I think 50-60 for 2 spots is an overestimation, but I also can’t imagine them interviewing only 10 people for 1 spot so it must be somewhere in between. It will also be program dependent.
I'm sure this is program dependent. 50 interviewed applicants would be a lot for any allergy department (especially if they try to have 4+ interviews per applicant). My program has 2 spots. In previous years, there have been ~25 people interviewed total. This year we've scheduled the same amount of interview days so I wouldn't expect them to interview more people than before. For a program with a single spot, I imagine they might hedge their bets with ~15 to be safe (probably places that match higher on their list historically may feel comfortable interviewing less).

You can estimate how many interview spots a program is offering by the number of interview dates. Virtual interview groups may range anywhere from 4 to 8 people with 5-7 being most common.

Not sure where else to post this but figured this would be where? Applying next year-can someone give me an idea of my competitiveness for places like Vandy Duke Yale Stanford. Goal is focus on adult allergy with strong clinical training

Lifestyle and income potential. Pvt practice A/I makes more than hospitalist (considerably more if partner) and much more than primary care and they work less. Medscape has A/I as best worklife balance of any specialty beating Derm (negligibly). Generally happy patient population with immediate results of your care and drastically improving their quality of life

We typically interviewed about 20-25 people for 2 spots. Both years I was there, we had 1 or more internal candidates from the IM and Peds programs at our institution. That certainly gives the PD some comfort for matching. Not uncommon to have sort of an unwritten pre match agreement to highly rank/accept one of those candidates too. PDs often have a lot of insight into internal candidates as well, ie they are from the area, own a home in the city, have a spouse who’s still going to be in training at that institution, etc. Sort of a sure thing.

Has anyone heard anything from Northwell, SUNY Downstate, or Rutgers NJMS?
No to SUNY and Rutgers. I did email Northwell and they said they’re still going through applications but I know some people have heard from them.

Its worth a shot, you have nothing to lose. It does feel like most interview candidates are finalized now though, I didn't get any interviews this week and less people posted in here this week that they got invites.

How many interviews are needed in order to match? Is there any hard evidence or data on this?
It's hard to pin this down in such a small field. I'm pretty sure there is match data from the NRMP for this for other fields. You can try and extrapolate but who knows. The past few years have changed the game because of virtual interviews. I think alot of places are back to in person now though but I'm not sure. Virtual interviews skewed everything because applicants had the potential to accept and complete far more interviews. Programs were also able to interview more. I think the top quartile or so of applicants really affected this because they were doing 20 interviews. Fellowship interviews are different than residency. In residency, when you're interviewing during your 4th year of med school, you can do a ton of interviews. If you apply broadly and are even moderately competitive, you are pretty much only limited by your willingness to spend the money....at least for large fields such as IM and peds. I couples matched for residency. My wife and I did somewhere around 20 interviews. With fellowship interviews, you are typically interviewing in your last year of residency. Programs expect this and try to accommodate but you still gotta work. Those ICUs and wards won't run themselves. Save for those in a very program dense geographic area where you might be able to drive to multiple interviews with a single day off work, most of us required 2-3 days off for travel and interview time. I think I did 10 A/I interviews and that was very taxing on my schedule. I ended up declining multiple offers. I matched at the 4th spot on my rank list and I would say I was a relatively competitive applicant. A/I also adds variables because it is such a small field. You might have 1 or 2 spots in a program and one or both might be basically "promised" to an in house applicant. It sucks but the whole interview is basically a facade. You also could be really competitive but some other applicant has one particular connection or quality that trumps the rest.

I'd say by the time you get to 8-10 interviews (assuming everyone is in person again), you are quite likely to match. However, this isn't based off data. You'll hear anecdotes of people with 3 interviews that match, those with 12 that didn't, and the suicide match that goes smoothly. The best advice I can give is apply broadly and do as many interviews as you can. Make the programs think that you really want to be there without looking like you're dishonest.

Has anyone on this thread heard from Stanford yet? Seems like last year's invites came out super late. And based on the initial email they sent out when we first submitted applications, I assume we would at least hear about rejection vs interview invite? Thanks!
 
Hi all. I'm thinking of cancelling the remainder of my interviews with a particular program. I had one interview last week, and have a couple (2nd interviews) next week. I'm likely not going to rank them (personal preferences) but just wondering if they find that rude since I've already had one interview.
