I believe this is Bofelli group. Obviously larger system but he is within it although I may be wrong but I know multiple podiatrists work for that organization. And again, we've said this. They're not looking to add more podiatrists they're looking to add more APP. This is the future.

And to be fair this is not something exclusive to podiatry this is all specialties. But other specialties don't have problems finding good hospital employee jobs.

We had two docs through the major hospital system in residency who had busy clinics that residents didnt frequently cover. They requested the hospital add a shared NP for clinic duties. Didn’t disrupt our resident flow one bit. I know we like to be doom and gloom but most places that I’ve heard doing this is at the request of the pods.

I believe this group is associated with a podiatry residency also.

Yes absolutely, that's what we've been saying on here. Podiatrists don't want to hire either podiatrists within their system they want nurse practitioners or PAs. That's why this BS about being a non-operative podiatrist isn't a thing. Other podiatrists don't want a non-operative podiatrist they want a PA.
 
Absolutely, nonop pod working for anything than a PP group doesn't make any sense because a system likely pays half to have an NP/PA do the exact same things. To reference my residency again, one of the other larger systems had a handful of nonop pods. I can't imagine the hospital doesn't look at the bottom line and think why do we pay these guys this much especially when they also don't cover call.
 
Admin at my hospital has made it very clear that they want me in the OR as much as possible. They tried to give me two OR days when I first started as a new grad lol. Should’ve taken it to have an extra day off

Same. I am 7 weeks into my new hospital job and I will be in OR 2 days a week and that is where they want me. Will be a NP within the year I suppose

You're not in a state, you're in IHS... they can do whatever they want. 🙂
I’m a bit worried about skill atrophy as well. I’m doing 1-2 cases a week but it’s almost all diabetic foot right now. Haven’t done a scope since residency and not sure how it would go at this point.
You should have no problem getting some trauma in IHS... did you talk to ER docs/midlevels?
Ortho is non-existent or very minimally at most IHS. Even if they are there, usually not go-getters to any extent.
I'd talk to ER/urgent care "providers." A lot of it will be diabetic trauma, but it beats 100% pus bus cases.
Do get ready for stupid questions on-call from PA/NPs and attempts to call you in for dinky things.
It will eventually get you to boards, though (trauma ORIFs lead to eventual fusions, scopes, etc for post-trauma OA).

The hardest part for IHS is getting the elective recon (many pts are bad candidates... obese, no transportation, subtance use, etc).
It exists, though. Talk to PCPs about what you can do. They know you can do wounds and ingrowns, but talk deformities/msk.
Double that rare recon cases for VA jobs as very very few female pts, no kids, even more psych problems.
 
At this place it’s a tribal position so it’s kind of like a regular hospital that mostly sees Native Americans (several tribes around, it’s probably 70% the main tribe). There is the occasional non Native coming to the ER in more remote areas. The facility I operate out of is actually located in a decent sized city (the capture area is 1 mil+ population) so I think most members of the tribes are used to going to outside ER for trauma or anything more serious than a cold. There is one pod that has been here for a few years but he only does diabetic foot cases and practices in the more remote areas outside the major city. He doesn’t do any trauma or really any elective cases other than the occasional bunion.

I have done a lateral ankle ligament repair, ankle fracture. Have also done a couple of ulcer offloading surgeries. Have a couple of 1st MPJ fusions coming up. Other than that it’s been many amps and I&Ds.

I did talk to the urgent care NP/PAs about sending me stuff but so far it’s been mostly diabetic foot infection stuff. Need to talk to their main ER docs about what I can do though, its in a separate city on their tribal land that I don’t go to often because the other pod covers that area.

I’m getting pretty busy though so can’t complain. Seeing about 12-18/day in clinic with 2-3 inpatient consults per week. 2-4 cases per week the past few weeks.

Promise you have many more ulcer offloading surgeries.... literally every ulcer. Has this been here before? Why is it here? How do I do surgery to get it to not come back.

An ulcer is a biomechanical problem with some open skin, complicated by infection, blood flow, nutrition and blood sugar.

To expand on this ...diabetic shoes only help if you can get them ordered, if the patient wears them... The benefit of surgery especially these days with MIS is you can make significant improvement with very minimal risk. The less you leave up to the patient and the more you can control the better long-term success you have for ulcer healing. Diabetic shoes are not the answer. Surgical offloading is the long-term answer.

I interviewed at medical group bought by CVS.... They started you at 39 for RVU. Bend Oregon. That money doesn't go very far there..

They would not offer that amount of money per RVU if they did not think someone would accept. Guess what? Somebody probably did.

Ortho groups know the deal. They are offering slightly better salaries and benefits then private practice podiatry groups but not nearly as high as hospital practices...because they know they can get away with it.

There are newly minted fellowship trained DPMs every year who are literally cold calling and begging current pods at hospitals for a job. That is how sad the state of podiatry jobs are.

We don't need this many podiatrists graduating every year.

Why don't you guys check out the ACFAS official job board and see how many Ortho jobs are advertised there and how many of pod jobs. That's all you need to know about this profession.

 
Why do they advertise ortho foot and ankle on that site?
Basically recruiters / HR posting the job who don't know the diff (post to ziprecruiter, indeed, DocCafe, PracticeLink, aaos, aofas, etc).
Acfas doesn't use the p-word, so I think it confuses them sometimes when they're shotgun posting a job.

Back when I was in the employed job market, I'd email on some and ask if they'd consider a DPM... none that I inquired on would. (but still work asing... once in awhile, one will take DPM app or interview if they can't get the MD/DO interest.. but most are firm on ortho).

Because they probably pay to post their jobs there that's all. All about the dollars

Single earner here with 4 kids. Luckily student loans have been paused forever and landed a decent hospital job (with loan payback). Also we don’t have expensive habits or anything because we spent the past several years poor.

At first I thought "partner" meant "business partner" and I thought yeah that makes sense, buy in with someone who's really productive and draft behind them, cool.

Anyway 2 divorces later, I've found that kind of partner less of a sure bet...

Linkedin, Indeed, and craigslist does not have podiatry physician / surgeon job offerings with competitive pay or benefits vs if I look up foot & ankle surgeon where there's a bunch but states (no DPMs). Are we not wanted unlike Ortho-Pods?

I can't help but wonder if general surgeon, ortho, plastics jobs have "visiting nursing homes and assisted living facilities" as part of a "surgical" job? Hmm...

People still read newspapers? In all seriousness though, how do you get a job in this field?
Some ppl say "word of mouth," but it's more just cold calling and emailing and asking until you are blue in the face... then dark blue... then purple... then bluish-black. Have fun.

I got sick of the crap jobs I had (very low pay for what I produced, crap/low staffing, middle of nowhere, crap job quality... or all of the above), and I eventually made my job.
 
There used to be a bunch of jobs available with Indian Health Service posted on their website but even those are mostly dried up at this point. There are a couple still listed that would probably pay >200k but locations are generally undesirable.

I have a couple of good contacts if you want to DM me for IHS jobs that are likely still needing someone because they have trouble filling positions.
 
There's a bunch of Kaiser jobs if you can afford to live in California without any type of bonus potential
 
Best opportunities for podiatry in no particular order.
1. Practice owner/partner
2. Hospital employed
3. Ortho or MSG
… I guess VA/IHS behind these

Even if you have one of the above there is no guarantee you make over 300k though. Likely after 1-2 years you will if you get production bonuses in the top 3.

Most of the employed jobs are already taken or you are competing with pods who have more experience than you. Job market sucks though and I really feel for graduating residents in our job market and economy.

I think your best shot at getting a good paying job right off the bat is find a community hospital in a rural area and get in touch with the CMO/CEO to see if they want to create position for you. Probably works better if you have some ties to the area.
