The local news came and interviewed me about the heat. It was 117 today in Vegas. They asked me about heat stroke. I said I was against it.

is this year over yet?

i'm sleepy all the time, i'm all call before i'm ever rested from the previous call, i have an hour or two max after i get home before falling asleep, i have no life because i'm never awake when life happens and i'm miserable because i hardly see anything or anyone except for work and cat 😡

All right, I can live with that, as long as I can get some much-needed IV rehydration therapy from some moderately attractive nurses while I'm waiting. I'm guessing 115 degree temperatures + EtOH consumption will not be a good combination in myself or any of the other thousands of tourists stopping by Sin City this weekend. :meanie:

See the above for why 2-4 programs are rough...

You take it correctly.. FWIW I prefer either the 1-4 or 1-3 programs just for continuity's sake and as a way to do even less floor months. IMO rounding on some pt for the 10th day isnt of much value for those of us who will be spending the rest of our lives in the ED.

Good point. Why is it that some programs want us to do FP for a year before focusing on EM?
 
Maybe they think we'll by some miracle have a change of heart and become FP docs instead :meanie:

It may not be a good combination but it certainly is popular.

I just saw a new record tonight for most recreational pharmaceuticals by one patient by history (pt's friends....he had the GCS of a lampshade), rapid screen and being independently verified by the lab.....the ED doc's words: "I'll pay for the damn tests to prove him wrong." How many drugs you ask?
 
11 by history......9 verified by the rapid drug screen
1- LSD
2- PCP
3- Opiates
4- Amphetamines
5- Ecstasy (pending verification)
6- Benzodiazepines
7- Mushrooms (when they lavaged him, there were fragments of what looked like mushroom in the vomit)
8- Marijauna
9- Acetaminophen (I know.....not a recreational pharmaceutical but it came up on the drug screen)
10- Alcohol (0.21 BAC)
11- Barbiturates
 
Oh, and apparently the amphetamines weren't doing it so the patient allegedly also took an "entire box" of caffeine pills. How this guy wasn't dead I have no idea....he was close though.....SBP of 75 and HR of 150 after a couple of liters of fluid courtesy of the medics (they weren't about to give him pressors because of the polypharm).

Man anyone else feel ******ed everyday they show up at work? I show up.. fell ******ed for a few hours.. go home with my tail tucked between my legs.. I guess thats why it is called education.
 
I felt kind of ******ed yesterday until I stopped an FP resident from killing an asthmatic patient. Then I didn't feel so bad in comparison. I also got to use the analogy of a dog humping someone's leg in clinical practice...... 👍 It was a good day.

******s are the only ones tat can be taught. The instant geniuses are dangerous to patients and need to be shown otherwise. Fetus, I notice that even thought you're in your internship year, you're still able to post regularly on SDN. Will we have free time in residency?

and either IM or surg or transitional. but seeing as there's only one transitional program in nyc....
 
fetus, i think i actually feel my brain cells dying every day. in fact i think the only thing that's growing in number are the papercuts from all the stupid paperwork.

I'd rather do a year of surgery than a year of FP or IM. But that's just me.....

I think you don't know what you are talking about. I DID a year of IM, thought it sucked, and STILL would do it again before doing 6 MONTHS of surgery.

FM has some variety, and there's only a month of surgery in the intern year, instead of 12 months of dressing changes, abuse, and no time in the OR.

yup. that pretty much is summing up my intern year thus far. 3 weeks down. oh, and the sleep deprivation with a cherry on top.
