I understand some residency programs have a PGY1 to PGY2 system for their PGY1 residents. However, do most programs choose from their own pool of PGY1 residents for their PGY2 positions or do they prefer fresh blood from other programs? TIA

Most programs bypass the PGY1 and PGY2 residents in favor of PGY3.

This isn't really an answerable question, friend. Some programs early commit, so clearly they choose from within. But beyond that, I don't think you can really say what "most" programs do. It's the same as applying for a PGY1 at a location you were a student. I'm sure your chances are higher because they know you, but that's not to say they're not considering other candidates.
(TBH though the transition is a heck of a lot smoother if the incoming PGY2 is from your hospital, so I can see the appeal.)
 
some program early commit, it make sense for the program and the person committing if half way through the year its already a good fit and the resident is progressing well. however if you're barely making it type of resident then its unlikely you'll be early committed. speak from experience.
