Question about TMDSAS Eval. Letters

So I was gonna add the physician (MD) that was shadowing for the past 2 years as an evaluator, but when I went to select the "Relationship to you:*" drop box, there isn't a practicing physician option. The only options are:

• Health Professions Advisor
• Academic Advisor
• Graudate Advisor
• Immediate Work/Volunteer Supervisor
• Business Associate
• Current or Former Professor
• Lab Supervisor
• Chair-Graduate Department
• Practicing Veterinarian
• Practicing Dentist
• JAMP Faculty Director (JFD)
• Research PI
• Other

As far as I can tell, I don't think any of these are really good matches. And it's weird since practicing vet. and practicing dentist are options, but not practicing physician or the like. What option should I select so that the selection committee knows that I shadowed under a doctor for 2 years and he's the one writing the letter?

When you select "other" does it give you an option to type in a title for the person?
