Hey Guys,

I was wondering is it necessary or "highly recommended" to do a Sub I rotation prior to applying to IM residencies? I did a 12 week IM rotation during third year and I did IM related electives (neuro,Renal, ICU) so far but I may not be able to do my Sub I rotation before Sept 1. Is this unadvisable? Should I restructure the rest of my rotations to include the Sub I before Sept 1 or is it ok to do it at a later date?

Did any of you guys apply to IM without having completed a Sub I rotation? How did this fare for you?

Also a question regarding LORs? I have LORs from my renal and ICU electives (obviously IM related)... do you think these will suffice or should I get a specific letter from a formal Sub I or IM rotation?

It is not 100% necessary to do a Sub-I but it will help your chances of matching into that program. Especially if the program is competitive you need to do a Sub-I there. It shouldn't matter when you do it as long as it is during their interview season and you can interview while you are there. Check out this post on how to do well on an IM Sub-I.

It's advisable to have at least 3-4 letters of rec with at least one in your specialty. You definitely need more than two, some programs require 3. Check with the programs you want to match into and see how many LORs they require.
 
IIRC, you are a Carib student. As such, you should have the strongest possible clinical letters in your file and that should definitely include a SubI as many programs place as much/more emphasis on your SubI performance (when you're supposed to be acting at the level of an intern) than they do on your clerkship performance (where you're still trying to figure out which end of the stethoscope to put in your ears).

Your LORs should come from your IM clerkship and/or your IM SubI. An ICU letter and an elective letter will also be OK but you need one from your clerkship or SubI (or both).

I would personally shift your schedule so that you can get your SubI in sooner. Since they're pushing the MSPE date up next year, interviews will likely come sooner as well and you want to have all the important stuff in ERAS when the MSPE drops.

A 4th year sub-I can sometimes be used to make up for a sub-optimal performance during 3rd year IM clerkship. It's not required certainly and looking at your schedule, they will likely put some stock in your ICU rotation grade as some people do this in lieu of a sub-I.
