Got a question from usmle-rx that asked which of the following medications would reduce the effectiveness of a flouroquinolone?
a. aspirin
b. calcium carbonate
c. cimetidine
d. misoprostol
e. omeprazole

I understand the calcium carbonate would reduce absorption and therefore the effectiveness but wouldn't omeprazole inhibit p450 and reduce the effectiveness of ciprofloxacin?
Does the fact that flouroquinolones also inhibit p450 play into it?

Any help would be greatly appreciated!
Thanks

Why would inhibiting P450 reduce it's effectiveness, if anything it would increase its half life (making the drug stay in the system longer thus being MORE effective). Additionally I would be more worried about Cimetedine inhibiting p450 before Omeprazole.

The only time inhibiting P450 would cause a reduce effectiveness is if you're taking a prodrug which needs to be metabolized to its active form through P450 to be effective.
