Applying next cycle (May 2024), What are some things in my app that I need to improve to be more competitive for T20s? Also, hours are projected by May 2024 not matriculation.

My motivation for pushing medicine is from encountering medical disparities, so all previous experiences have been centered around serving the underserved community in which I live in, primary African American and Hispanic American populations. No heavy preference for state or region, but would like to attend medical school in urban setting.
I don't really have a school list yet, but so far I am planning to apply to: All Michigan schools (Wayne, Central, Western, Oakland, Michigan - Ann Arbor, MSUCHM), Some research heavy schools (Einstein, Cleveland Clinic, WashU, Mount Sinai), Howard, OOS friendly schools, and a couple reaches (Penn, Yale, Pritzker, Mayo, NYU, Duke, JHU)
Mainly need advice on possible improvements to make and a preliminary school list
