You have taken neither the MCAT nor any science classes yet...It will be hard to say if you can get into any medical school yet let alone a T20. If you can do well on those things you could have a compelling application.
There are plenty of people on these forums with 3.9, 520 + scores who don’t get into a single med school and have to reapply. I volunteered this past year with a student who had a 4.0 524 and is going on his second cycle of applications.
Instead of worrying about t20 chances, focus on doing well, taking your pre reqs and getting into any med school
