Big question I have is does only a 3 point increase hurt me, do I still have chances at t20s ? Any advice/thoughts would be appreciated!

When did you take both MCATs? A 3-point increase is within the standard deviation of re-takers, and it is thus not considered that impressive. Note that many schools average scores for screening.

I also think that your lack of community service orientation activities (food distribution, shelter volunteer, job/tax preparation, legal support, transportation services, housing rehabilitation) may also work against you. 500 hours of teaching people is a reflection of an academic life, but not necessarily one where you are comfortable in communion with those unlike yourself, who are chronically ill, or who are suffering/in pain. Thus, not sure about Einstein, NYU, UCSF. It looks like you threw in Virginia Tech, Temple, NYMC, Wake Forest, Penn State, and Miami; what do they offer to satisfy your interest in policy?

You have 600 hours in health policy, so I'm guessing that's why you're only interested in T20s, yes? I am sure you would still include the DC and MD schools. To that end, if your work is interesting, it might help you a little.

Every medical school is going to train you to "serve the underserved" to the point it is rather cliche among med school applicants. You have studied the issues from an academic standpoint, so I'm not sure how willing you have been/will be to be vulnerable and get dirty experiencing the issues. No primary care or emergency medicine. No disability or neurodiversity is apparent in the communities described in the WAMC profile. If your PS has a strong narrative of helping the underserved, this WAMC profile has completely sanitized it to me.
