Go for it! There's no harm in trying. If you can simply be certified/eligible (which is just filling out that form), then it can help get interviews at participating WICHE receiving schools (acceptance is more up to your own merit, but it will help get interviews). AZ doesn't reveal how they select recipients (although I suspect it's stat-based, for the most part). If you get the funding and go to one of the WICHE receiving schools, it will knock $100K off your tuition (currently, AZ gives about $25K per year towards tuition). For AZ, you are required to practice for 4 years in AZ in exchange for the funding (or 2 in an underserved area in AZ). HOWEVER, even if you're not sure where you want to practice, just go for it. The worst that can happen is that you get the funding, don't stay in AZ, and then you are required to pay the money back....at a whopping 0.5% interest! So it's a good deal no matter how you look at it. If you've got decent stats, I'd suspect your more likely to receive the funding. Also, AZ must have just done something, because apparently they got more funding for the program. A classmate of mine who didn't get the funding back at the start of school (2012) but applied for it when I did just got an email asking if he wanted ~$25K to go towards his fourth-year tuition. Some kind of retro-active something or other because they have more funding. AZ is the biggest sending school for dental. So just go for it.
