BU internal medicine does still have a reputation for being malignant, and yes they do find ways around the call schedule, the resident class is huge and some do get the fellowships they want, but some do not maybe 40%?, so there is alot of copmetition, many of the residents that I worked with were *very* stressed out about getting their fellowship, because after working the disorganized and time intensive setting at BU's older hospital realized that primary care/general internal medicine was not for them. (i.e. realized that they could not keep their sanity as an general medicine intensivist). If you can I would go to a place that is more research oriented and has better connections for fellowships then you wouldn't have to stress out so much. (Getting research is hard with the hidden time costs of being a resident here) I would definitely try to get the real story from residents that are in the program when you interview not the handpicked residents, . . . it is a difficult environment to work in if you want to provide the best clinical care as there is alot of politics and the teaching on the wards is definitely not the best available to most residents.
