Hi,
Can anyone tell me how to make amalgam class II MOD in lower first molar & replacing both the lingual cusps?
Its kind of urgent so please tell me ASAP. This is asked in temple preclinical simulation.

Hi,
as i could see that u r very sure about the question i.e., MODL on that tooth. The treatment options as far as i think
1. Lingual crown build up with amalgam by using the retentive features such as pins.
2. Onlay.

i think those are the opnly ones i can find and if they say it has to go for a Root canal treatement, then the other option is the Post and core with crown.

I dont know how much i healped you i this.

Even i think that it needs additional retentive feature on lingual side like pins, retentive grooves or detinal chambers. It should be ideally a case of onlay but i have no clue y they are asking for amalgam cavity prep? With amalgam i think chances of failure of restoration will be more.🙄

lingual cusp of mandibule is a non supportive cusp,the best bet,as krishhh said, will be pin and retention grooves and build the cusp up with amalgam,if they insist on having the tooth filled by amalgam.

if any situation in which there is mesial and distal caries and more than 1/3 of the bucco-lingual width of the crown is involved indication will be for using onlay,but where the buccal and or lingual surface are involved ,in this case 3/4 crown must be used.

What you're describing doesn't sound to me as an amalgam prep, more like an onlay or crown.

You could try to find some Temple student in the Dental forum and ask them about the school's procedure in this case. Each school varies in its teachings.

Hello pkowe,
INDICATION: Amalgam MOD with cuspal reduction is mandatory when caries involves 2/3 rd distance from the central fissure to the cusp tip.
PROCEDURE: Prepare an MOD amalgam and reduce the non supporting cusp to about 1-1.5 mm and functional cusp to about 2mm so that the lingual / facial wall of the occlusal step is conserved for retention.
ADITIONAL RETENTION: Retentive features like slots and pinholes can be added.
I dont know whether they will expect you to prepare the additional retentive features.If I were you I will just prepare the MOD with cuspal reduction.

I think they want to know if you know how to make a retentive prep.
You might want to include placing trough/ groves at the DEJ in your answer. If you are placing pins, one pin for every cusp that's missing.
