I would buy them a card.... and then inside the card put a check for about 20K or so, that should cover the first year.

Actually, why don't you buy them like a model Eye or something? That would be kinda cool

Try some searches on the internet using optometry gifts, eye gifts, medical gifts, professional gifts, etc. Perhaps an antique pair of glasses? Or get creative and make your own eye chart (with a special message?)
