I know several docs who banded together and formed a new company when my group blew up, hilariously they were terrible at onboarding and took weeks and even months to get credentials to work. At hospitals they already worked at! They still can’t seem to get a grip and spent a ton of $ to get on whatever vendor system.

My point is, don’t go with that kind of an outfit. Use a larger more legit agency that can help you especially if you run into issues like shifts suddenly being cancelled or payment disputes.
 
Look up flex health solutions/now edgeford. Look who runs it. Look who the front person is…and now the husband is “helping” her run it

This is a story as old as DEI has been around.

Using minority women as a front person for a company to get contracts. Not the first nor last one to do it. It’s just not the locums business.

My crna friend (African American) woman who’s former military been telling how this is the angle people use to get contracts. I wouldn’t expose all this unless people are educating me.


LinkedIn says husband who is also black is cofounder/CFO. Is he also a “front person”?

The reason I know who front these companies because my colleague who is about to inherit over a billion dollars (only child). Her mom was minority contracted award winner for a huge govt contract in engineering. And her dad ran the company. Yes her mom knew stuff also. But her dad ran the company.

You use the front person who gives you the biggest advantage.

Now companies are less likely to consider this when awarding contracts. Just the way the world works.

Aneftp says some kookie stuff on here, but he is not wrong about this. For years at the VA when you would go to buy something, like a glide scope or an ultrasound, we would work directly with the manufacturer. When the purchase approval went through all of the channels, we inevitably would end up buying these items, not directly from the manufacturer, but through distributorships that inevitably listed on their letterhead things like disabled veteran, Native American, visually impaired, female owned company. Reasonable people can debate whether this is good or bad, but that’s what happened.

Tuesday-Thursday 7-3 ($300/hr)
Monday and Fridays 7-3 ($325-350/hr).

This pretty much disincentivizes those who want to take long weekends.

After 3-5pm. Pay them $25/more per hour
5-7pm pay them $50/more per hour

Call weeknights starts at $400/hr in house
Call beeper starts at $350/hr regardless of cases or now. First 8 hrs are guaranteed. Call back is $450/hr (pretty much pay someone $2800 do no work) if they don’t get called in but if they get killed last my colleague Sunday night working 7p-5am (they would get $4500 for the call

Weekends days start at $400.hr
Weeknights starts all $450/hr flat

This type of uber surge pricing will work. The lazy docs who have aged out of calls will still make 500k/42 weeks no calls no weekends.

The semi taking call docs will hit 700k/10 weeks off
The heavy hitters will make in excess of 1-1.5 million depending how much calls you take

Eliminate the locums company.

It’s so simple. Yet administration doesn’t think.

Now you will say some scammers will just try to work elsewhere for $400/hr daytime and try to grab the night calls to maximize their pay. That’s fine. But you give the docs who start the day off 7-3 the opportunity to continue working the full 24 if they want first.

Obvi there is great debate about actual $ amounts.

But that aside, I agree with your premise that institutions need to properly incentivize less desired work times. Blue collar hourly jobs have been doing it for decades with those shift differentials.

And what happens every time a company / hospital messes with those differentials? People leave or say F-it and switch to better schedules ASAP. And they can because the good ones have seniority so they are first in line for open positions.

Employed physicians are just too stagnant to respond with their walking feet. But the ones that do make out like bandits. In non-medicine switching companies often results in a quick 10-20% salary increase if there is demand.

Right now there is demand for our skills and services. Folks need to wise up and see how much their employer is praying people don’t leave. Likewise, that means other employers are praying quality docs are willing to switch jobs. One decider - $$$.

Just my opinion. Nights weekends and holidays should pay more.
I get some like nightcall and getting paid to mostly sleep with occasional sleep disturbances.
I value my beauty sleep and would rather work 7a-5p and get 6-8 hours or undisturbed night sleep. Now doubledipping and taking nightcall to double the daily income is enticing. 4k daytime and 4k nighttime especially if getting paid to mostly sleep.
If I had to choose between 7 a-5p or 7p-7a coverage for 4k I would rather take the 7a-5p everyday.
Now if I had a business or other activity I was heavily involved in during the daytime then taking nighttime makes more sense

The premise is solid, but how do you guarantee every call slot is filled? That's the issue as you can't have nights when no one is on call

Release the schedule 60-70 days out. Plenty of time to fill in the holes. Since every one is independent contractor. The professionals ones always map their schedules 60 days out.

Sure there may be holy holidays weekends harder to fill. But plenty of time for people to plug in the holes. Within 20 days put in other incentives like 1.5x. They will immediately get filled.

But it takes an administration with vested interest. You have people who aren’t vested doing the schedule

Does it matter who is using who? They got their contract due to their minority status. That's the whole point.
 
What's up with some of these places posting jobs for weekend shifts? One I saw in Houston was 48hrs (Sat-Mon) for flat fee of $2,000 plus low call back with call back $350/hr pay. That's $41.66/hr for 48 hours with the low chance that you'll get the $350/hr call back. Or I saw a Florida weekender posted for $2,500 for 48hrs and then $350/hr low call back place. WTF? Are people actually accepting these offers to be on standby for $41.66/hr and waste weekends? You could get an overtime job at Home Depot or be a waiter at a restaurant and make more. CRNAs would never take such a pathetic offer

You would be surprised. Generally it’s someone who just has a free weekend locally and dosent want to work but a free 2500 sounds good. I know a pediatric guy that takes “call” at 1000/ nights at local university system( plus call back rate) Last month he took call 12 times and worked zero hours. 12k for doing nothing. There are no emergencies so he knows well in advance if he will be working. He can pretty much live his normal life and get paid extra.

2K per call shift + 500/hr callback rate.
usually high paying jobs like this get you in.. and the minute they get somebody cheaper... you are gone.. cancelled. could be 2months.. or 2 years. you dont know until you get your 30 d notice.

You are operating out of fear. Fear of the unknown paycheck. The sense of security a w2 paycheck gives you.

Ok…Locums is 30 days notice (sometimes 60 days notice with longer term 1099 contracts)

But this isn’t 2013-2018 or whatever frame ur local region job market became tight. Where there were 10 applicants for every one w2 job available

One of the locums places (and I have tons of privileges) said in 2017. They had 19 crnas apply for one w2 crna position. 19!!

Now they are short 6 fte crnas.

So you operate out of fear. There is nothing to fear. No w2 job is safe either outside of a VA job after probation period. Even state academic jobs can give you a notice of non renewal (a backdoor way of terminating you without saying you are fired)

And most standard contracts are 90 days without clause termination.

The key to locums is maintaining privileges are multiple places. I had 3 locums places terminate me in the last 90 days. But I also picked up another hospital to get credentials. So I’m still at 9 hospitals locums and 3 surgery centers. This is the game you gotta play. Sure it’s more work to do and maintain the privileges. But it’s worth it. I have two state medical licenses (Virginia and California) I still pay for. Because they are not interstate medical licenses. It’s insurance policy for me if the job market gets tight in Florida. Because I can work in those two states faster than someone who doesn’t have a full Virginia or California medical license.

You gotta have a plan when you do locums.

And no. Your general pay won’t be up 50% if everyone leaves. It’s a bad way of thinking. What happens if they will make the current w2 docs suffer and over work them. The longer you stay. Let’s say admin says change is coming. If you stay 18 months. You gave admin extra money you should be making elsewhere for your workload.

So the locums docs are making 800k no calls at the same place you are w2 making 550-600k with calls. Admin after 18 months says. Here is your 50k extra pay raise. They aren’t gonna to say here is your 200k pay raise.

I can sign up my PRN shifts next week and my locums 30 days out on a 30-day termination notice.

I don’t consider this as highpaying.
2k for call is low in my opinion.
Crnas make 4k for light ob call

You're not going to find 425 in any major metro reliably, including NY, Chicago, LA etc.
2K beeper is pretty good actually, since you are getting paid the hourly on top of that. The median for this based on prior shifts with hours worked is about 6-7K. Regarding the CRNA OB Call, yes they can make that much, but they dont make hourly on top of their call pay. And they have to deal with OB.

Chicago has had jobs paying 450/hr for general anesthesia.

400/hr can be found pretty easily in most major cities. The premium for rural should be at least 450/hr if not higher.

I did locums for 11 years exclusively. I havent been a locum for over 10 years now. I have heard it all. Ive been told every lie there is by the agencies and the employer. Ive been cancelled while i was at the airport boarding the plane going to the location. I have had my assignment cut short because of no needs because of hiring. Ive been left in the cold for 6 months or greater. Dont get me wrong I had PHENOMENAL years financially as a locum but towards the end of my stint as a locum I really got tired of being told lies. It is such a unsavory business.

Also, you kinda get a stigma associated with you when youre a locum. not that it bothered me but back then being an exclusive locum you were less appealing to jobs when you applied for them. I had big name places turn me down because I had too many job sites; it was a red flag. Even though most places that i went said i was a top-tier locum. I did everything.

This means you are in a good place in your life not to hustle anymore.

And yes. That 2012-2017 era in many cities with almost every profitable group selling out and the monopolistic behavior of AMC meant less and less truly desirable jobs. A lot of people got very anxious who were doing full time locums back than. Mostly because they also didn’t want to work in the boonies 1099 either. They wanted to stay in bigger metro areas
 
Chicago has had jobs paying 450/hr for general anesthesia.

400/hr can be found pretty easily in most major cities. The premium for rural should be at least 450/hr if not higher.
I can confirm this based on what friends have gotten in the Chicagoland area.

I did locums for 11 years exclusively. I havent been a locum for over 10 years now. I have heard it all. Ive been told every lie there is by the agencies and the employer. Ive been cancelled while i was at the airport boarding the plane going to the location. I have had my assignment cut short because of no needs because of hiring. Ive been left in the cold for 6 months or greater. Dont get me wrong I had PHENOMENAL years financially as a locum but towards the end of my stint as a locum I really got tired of being told lies. It is such a unsavory business.

Also, you kinda get a stigma associated with you when youre a locum. not that it bothered me but back then being an exclusive locum you were less appealing to jobs when you applied for them. I had big name places turn me down because I had too many job sites; it was a red flag. Even though most places that i went said i was a top-tier locum. I did everything.
Yes, but that was when locums was not in as much demand as it is post-covid. the entire market dynamics have shifted.

Plus what am I gonna do with the money? Its all going to someone when i drop dead. Im almost at 8 figures.

Why would half go to the man? I would expect 35%-40% going to the man as a W2. My effective tax rate is lower as a 1099 doing locums than as a W2

I agree, I got sick of locums and I didn't do it near as much as you.

Why don't you retire? Unless you're doing this out of boredom or the love of anesthesia. But I don't get the impression you love to work.

Spend some of that money. It's enough. Travel, do what you love.

But being left in the cold for 6 months back when job market was much tighter is a combination of not having multiple side gigs going on at the same time. Don’t be a one trick pony.

But if you feel safer as w2. That’s fine. It’s ur business and no one will fault you for that.

But it’s 2026. Remember that. There are so many locums jobs out there. I’m always being asked almost every week if I’m available for 1-2 1099 shifts they need coverage. I run in a social circle where we are all friends and we cross cover for each other in the same 1099 gigs.

Seems like you were the lone wolf and got caught in a depressed market.
I think it depends on the w2 jobs and market as well. In south Florida with envision there is a job opening at one of the hospital every few months and that’s an underestimate. Even if locums doesn’t pan out there is usually little worry to find a new w2 job or per diem work. Unless the w2 job is really amazing but it rarely is with these AMCs

The calls especially Fridays Saturday and Sundays are where w2 docs get hosed.

If places that have a hard time hiring w2. You incentivize the weekend calls first.
Basepay no calls no weekends. 4 days a week at 500k/42 weeks of work is really only $300/hr

The key is the day off each week if the doc is gonna to work 42 weeks.

Pay for the weekends

Some places are already doing that. It lifts morale knowing you get a little extra on weekends but it’s not forced on you to do mandatory nights or weekends.

Agreed. The only locums that get screwed are those that only have 1 job lined up or privileges at one hospital.
I can sign up my PRN shifts next week and my locums 30 days out on a 30-day termination notice.

You're not going to find 425 in any major metro reliably, including NY, Chicago, LA etc.
2K beeper is pretty good actually, since you are getting paid the hourly on top of that. The median for this based on prior shifts with hours worked is about 6-7K. Regarding the CRNA OB Call, yes they can make that much, but they dont make hourly on top of their call pay. And they have to deal with OB.

You get 2k to be on for 24 hours. most will do about 10-12 hours of work. I believe there is 3 hours gratis. but the rest is 500/hr. So for example, if you work 11 hours of those 24, you get 2K + 500x8 = 6K.

I'm not a fan of the gratis hours, but they do pay overtime rate from the get-go.

I’d avoid taking gratis hours. Defeats the purpose of the stipend holding the pager.
