I hope to get into the MD/PhD program at either Harvard or Stanford. Is this a realistic goal?

Science GPA: 3.8
(top 40 national university US)

First author publication in Nature Communications
Fifth author publication in Journal of Controlled Release

First Place Winner AAAS Poster Competition, featured in Science

Poster Presentations at Society for Developmental Biology and Experimental Biology

Hours shadowing: 50 (cardiology and NICU)

Volunteer: 200 hours at homeless shelter

Vice-President of Christian Fellowship on campus

Interned at pharmaceutical companies performing antibiotic development and epigenetic oncology research, and lead independent research effort culminating in first author publication about regenerative medicine research

Does anyone have thoughts on whether these programs are realistic targets?

I think you have a very good shot at interviews from the top places. Chances of getting admitted are not something we can comment on beyond that though, since your interview determines a lot.
