I never knew about the absolute incompetency and waste of federal work and spending until I did rotations/extenships at VAs.

It’s a necessary service. And I hated how the people who actually needed good care (veterans) the most were trapped behind not only horrible admin but once they got seen they were treated by a student instead of a proper doctor. If they were lucky they could talk to a resident. If they were fortunate enough at one of the programs maybe a fellow would see them.

As far as attendings go? It’s a “hi” and a “bye”

There will be wailing and gnashing of teeth in private practice if the VA is shut down. VA, Tricare, etc ie. Triwest administrators and other ilk are as bad as the worst Medicare Advantage plans. They can unilaterally cut your rates. They require all manner of prior authorization garbage and referrals. They take forever to pay. Novitas looks like angelic boy scouts next to the administrators who run government military insurance. I know I teased the NY guy who loved Medicare, but we'll be begging for plain old CMS Medicare with the plans that Trump's merry band chrony capitalists have in mind for government insurance.

Sooooo relatable. Weaponized incompentence. My people come to me with problems and I'll be like, "the only thing I can do about this is call up tech support/our accountant/the electrician and figure out how to resolve the issue, and that's the exact thing I'm asking of you."
 
Sorry, I may be a little too "online," but I haven't worked in 2 months and have another month to go.
 
I don't understand why our profession agrees to take call without pay. A lot of the stuff we do can be done at surgery centers, except for some of the big stuff maybe. I don't know how it got to the point where we are the only specialty that take call without pay. I never knew this as a student and recently learned this as a resident and it's extremely frustrating.

It's called saturation

Everyone is just dying for whatever volume they can get their hands on.

If you won't do it the CEO is going to find another crusty DPM down the road to do it. Or maybe the new fellowship trained DPM who is starting their own practice.

They would not do this if they did not think they could get interest for the position. We are so saturated and some podiatrists are so in debt and desperate that they will look to do anything.

They get cheap labor. String them along with promises. Never deliver. Rinse and repeat.
