Based on my experience this is not always the case. I’ve been told by a couple VA physicians (including a COS) that they have the power to “direct hire” as they please. In fact, the last 2-3 DPM openings at the VA I tried to get in with “direct hired” all their openings… none were ever seen on USAJOBS. 🤷‍♂️

This is why I finally gave up and moved on… was blessed to find a much better opportunity anyway 😎👍

I need to rant for a second. Currently in the job searching process and let me tell you I want to so badly put some of these practices on blast….its like they think I’m dumb. Sad thing is they rope people in who don’t know any better.

For example, if I reach out asking for a job opportunity within your practice and you’ve recently hired several new people, saying you’ve hired several new people and will keep me in mind is not wrong. Not telling me about those hires and wanting to continue to talk about a possible job when those new hires are NOT busy is and just makes me think you’re a mustache pod. And as I reached out to friends of mine who know the person who runs the practice, it’s nothing more than a scam podiatry office that takes advantage of new hires. I looked up for myself so I didn’t just take the word of someone to see how “busy” these new hires are and they have openings out the wazoo. How are you looking to add ANOTHER associate when your two new hires have open schedules every single day all day every week for weeks on end? Where’s the business? How are they supposed to make money? Meet their numbers? Actually you know practice?

Another private practice guy pretty much tried selling me a position that was nothing more than residency 2.0 with no room to negotiate anything about the contract because “the contracts are all the same in the area.” Like no sir, working a bazillion hours a day and taking call while making the same amount of money as someone who is working less is not “the same.”

I’d say it’s all comical at this point if people weren’t investing 6 figures to go to school for a career in this profession.

On the bright side, I have talked to a few people who seem genuine and we shall see where those convos go but my gosh the amount of scammers (that’s what i’ll call them) out there is a lot.

Can you tell us more about what type of surgery is done there?
 
Hey Air bud...longtime.

In Alberta we have full podiatry scope. So in theory/legally, anything foot and ankle. This will obviously depend on what site your surgical privileges' are at and if the surgery is private or government funded. With that being said, nobody that I know is doing rearfoot/ankle trauma or ankle scopes.

For me, it's 98% elective surgery, mostly forefoot with some Brostroms' and Achilles. The guys who work out of the larger hospitals focus on diabetic limb salvage.

There is only a hand full of DPMs in my area serving a population of 1.5M. Most of the DPMs are not surgical, so our practice stays very busy.

Why would you ever turn away refers?
Just schedule them out further and further.
Once you refuse them, your office is dead to patient / PCP.

You want that goodwill and refer stream once you get another associate (or if refers ever decreased).

Those are my concerns as well, but we really have no good immediate options at this point. We are already booking new patients out 3-4 months. The referrals just keep coming (not a bad problem to have!). Talked to my clinic manager today and she says that we get a steady 30-40 referrals a day. We have around 1200 PCPs that refer to our small group.

The referrals that we are sending out are for "diabetic foot checks", "routine foot care", and "nail fungus" as of right now. There is just no room in our practice for them.

She knows the area in which she wants to live and has reached out to a few private practices. The long term goal would either to either become a partner, or start her own in 3 years. I know zero about medicine but have been an entrepreneur all my life.

If she were to offer to a small private practice to earn 100% of her income from billing, what would that look like? What percentage would get in the ballpark? What kind of contractual language does there need to be for a "buy in"? Does the value of a practice typically get based off of revenue or ebitda? She is debt free and initial income is not as important as long term opportunity. There are many, many, smart Podiatrists, but growing up in our household she has a business sense that I have not seen in many of her peers.

You are essentially asking - could she do a no base, straight collections salary. I used to recommend it. I don't anymore. I could go on about this for an hour, but for starters - podiatry practices routinely obscure and refuse to provide actual associate collections. I've spoken to plural friends who can't tell me what they are bringing in recently.

In that situation, she'd be best off just starting her own office. No doubt.

It'd take a bit more marketing or time to get busy if the area's saturated (most are), but it'll happen fine if you're paying for her school/biz/living and she is not on a ticking clock for $$ and profitability. It usually works out fine anyways, but some pods have a lot of student debt, limited funds/credit, high interest or even hard money loans, etc that they use to open. She'd sidestep all of that stress. Plenty of pretty dim witted DPMs have done it and turned out fairly well. Sounds like she'd do great if your eval is reasonably on point and she is outgoing to meet local refer docs. Podiatry is a very red ocean business... much moreso than most other "doctor" types. Chiropractors might be the only ones more saturated. We are all "podiatric surgical residency" now; it will get worse with recent new podiatry schools and more grads. That could work to her advantage in the long term if she eventually wants to hire associate docs and do multiple locations (scalable podiatry biz... most profitable type, obviously).

It is fine to work for a year or two in the target area to learn (if there is no non-compete for docs!) and to get onto some insurance plans, meet some refer docs. I would do that if it's an area without physician non-competes (consult area employment/medical attorney). It will obviously make an enemy of the hiring doc/group when she leaves after a year or 2 or 3, but that typically happens a bit anyways for nearby docs/groups (competing for same pool of patients/refers). There are advantages to that brief employee route: get ideas, learn the area, make mistakes on someone else's dime, avoid the double pressure of first year(s) of patient care without training wheels - on top of running a biz from day 1. As you probably know, it will honestly make her appreciate the eventual freedom more if she has a crummy underpaid podiatry associate job first.

Buy in/out idea on podiatry offices is really tough, though... very tough to eval value, tougher to agree. Tougher yet to stay agreed. I've seen countless deals fall through, seller try to change price/terms after the deal is in process, seen big/small doc group partnerships break up even after years of working together fairly well. No reason to do any of that stuff if you can avoid it.

Most podiatry practices are generally worth about 1x NET (which is hard to eval)... but like anything, they're only worth what someone is willing to pay and can pay/finance. It was usually 1x gross or other stuff in the past, but value is not as great. Our equipment is very cheap relative to dent or other procedure docs. You are basically just "buying" the office's regular patients and goodwill and reputation (and real estate if applicable, which is pretty rare for podiatry). A lot of the patients come in, get treated, heal or leave, are done coming in fairly fast after they start... so valuation of office is tough and typically subjective. The only real ongoing podiatry patients for years and years are toenail/callus and diabetic foot patients, which are mostly low value visits and not too desirable or hard to acquire anyways.

There is nothing wrong with starting up an office and having it run her way... and pick her staff, her strategy, etc trouble-free. I personally looked about a dozen buy in/out opportunities over the years... all grossly overpriced or fell through for various reasons (owners typically would not make a defined exit plan, even if I was willing to overpay somewhat). I did solo instead, and it was more profitable and faster growth than I'd planned. In hindsight, I would have been a fool to buy out an office and basically take on another debt that is mortagage or podiatry student loan sized. No thanks.

That was on here months ago. I turned down an interview there in person. It's not 355k. The recruiter told me it was a super busy pod who was in the OR 2 days a week was relocating due to family needs....In reality it's a nicer older pod who is retiring and is mostly non surgical. He didn't know the salary. The admin I talked to initially on the phone didn't know the salary. BAU for Jackson research as others have previously stated.

Also get this....ME, a dumb podiatrist was able to ask for more money/signing after being offered a job...and they said yes.
 
That was on here months ago. I turned down an interview there in person. It's not 355k. The recruiter told me it was a super busy pod who was in the OR 2 days a week was relocating due to family needs....In reality it's a nicer older pod who is retiring and is mostly non surgical. He didn't know the salary. The admin I talked to initially on the phone didn't know the salary. BAU for Jackson research as others have previously stated.

Epic is great. Photo upload and insertion is great for wound progression and CYA
Everyone loves Epic but me. Its OK. Not amazing like people boast how great it is.

It's too complex and too many options. Requires a team of people hired in the hospital to help dumb doctors like me understand how to order something. I tried to order a MRI to a university hospital recently/outside our system and Epic couldnt find it. Just one example of how its not that great. Took a team of people to figure out how to order it. Pretty dumb because this isnt a small hospital.

The Haiku mobile app is alright. Could be improved upon greatly.

The photo upload option is nice.

Connecting EHR to other hospitals who use EPIC is the biggest plus IMO.

But maybe I just hate EMRs because in the past I used to complain about Athena on here. Athena works OK but its looses a ton of money to any company that uses it. When we adopted it at my last job we lost ~20% of collections once switching to Athena (from NextGen which really was a good one minus a few nuances).

Also I like Cerner. I think it works well. So what do I know lol

Bro that's why you just make an external referral and put in the comments where it needs to go and have other people do it. But yeah, bringing up another doc NEVER works.

When I do that it goes to an invisible bucket that never gets worked on.
Thats probably not an epic issue though. Thats probably a dysfunctional hospital issue.
 
Had one hospital in residency that had Meditech - that was painful.

Yeah, any referral I put in goes into a work queue for our referral coordinators. I’m sure it’s easier for them when when the office/provider has all of the info pre-populated into Epic. But when it’s not, they still get the order and just have to google or call the office for fax and referral information. I don’t think you’re experiencing and Epic issue, but I guess all of the builds can be a little different…
