I'm actually surprised at the number of new osteopathic residencies in 2015. Seems like a lot of schools are keeping up with their promises of having residencies for graduates. Hopefully most of these residencies survive the merger. Overall, excluding TRIs there were 123 new osteopathic programs that opened in 2015.

Probably 80-90% of them. The ones that might not survive will be the specialty ones- ortho, neurosurg, etc, where they might simply not have the case volume for the residency to continue. Hopefully they factored that in before applying for accreditation right before the merger though lol.

KC Care Clinic is a free clinic here...I seriously don't know how that became an IM residency program haha. I feel that one may close.
 
I'm surprised not to see any OB, though I admit I'm not as familiar as I should be with the nationwide residency expansion climate. Maybe Family is meeting the need.

36 approved positions- with that many positions, they have to be working with a hospital, but judging by the looks of things, the program has been put on hiatus. I'd have to look through a few of the other approved programs to see just how many are approved but not actually bringing residents onboard anytime soon.

There actually are five new OB programs, but I'm so OB averse my mind subconsciously avoided copy+pasting them. So that's five programs I failed to count to add to the numbers as well.

Looking through the program links provided, I find it interesting how many programs are approved but not bringing people on for a year or two. My bet is that they're going to put in their ACGME applications first, before bringing on a full set of faculty and staff.

Would you happen to know off the top of your head what the ACGME requirements are for faculty to resident ratios in Ob?

It's a smart move for them to wait to open these programs up so that they don't screw residents there.

As for the KC Care Clinic, I bet the faculty at KCU had this all set up before the merger deadline set in and realized that that wouldn't pass muster now with ACGME.

Given the hospital size, I'm not surprised. 222 beds and 140 physicians would make it hard to maintain and ACGME accredited residency in regard to faculty and the like.
 
The number of specialty openings is a little surprising. Hopefully they planned for ACGME accred beforehand like @Mad Jack said haha

A lot of free clinics- aka Community Health Centers- have started residency programs. In fact, there is federal funding through the HRSA (part of the Dept. Of Health and Human Services.) Both AOA and ACGME programs have opened under this funding program. These residencies have to meet the same accreditation requirements as any other residency. Most of them have inpatient rotations at local hospitals; the major difference is that the CHC is the home institution rather than the hospital.

So I'm assming you spend the majority of your IM training in outpatient right? It reminds me of a variation of a primary care track. Except with a free clinic there's a lot of variability.

It's probably safe to assume that these IM programs have a primary care focus, sure. Doesn't mean you can't be a hospitalist coming out of such a residency, though. Fellowship would probably be harder, but that's true of most community IM programs.

Will doing primary care track IM residency significantly lowers one's chances of matching a fellowship position afterwards?
 
Not necessarily. I believe a PC track requires 1/2 of the training to be outpatient, so this might affect your elective months, but many people have gone on to fellowships from a PC track.

Good to know. I assume that it's maybe a little more doable for non-procedual specialties like (Endo, Rheum, Geriatrics) than GI or Critical care.

There was some news release that I think ACGME is now granting across all programs that they will accept COMLEX. I can't seem to find the press release though.

Usually the community programs pair with a local hospital to meet these requirements, the same as ACGME community teaching center programs do.

They won't. DO schools are "okay" with pumping out more PCPs.
I can see most of the ACGME programs just saying "fine, then we require both COMLEX and USMLE then". Lol.
I can also see some of those programs toss any application with only COMLEX scores to the trash before it gets to the PDs' table.

That's a fair statement. Research time is another important consideration, especially in 2nd year.

Seems like progress I suppose, but it's really just mumbo jumbo that doesn't mean much. I'm assuming PD's will say " While the USMLE step 1 is not required for Osteopathic Students, it is highly recommended"... or something similar to that.

It would be great if OMM was a section added on to the USMLE and we would only need to take one test. Would save students a lot of money. Money gained for us is money lost for others.

As nice as it would be to only take one test, I think taking the USMLE is more important now than it ever has been. One match means more competition. Best way to be competitive is to take the same test everyone else does. Just my opinion

Pretty much. The merger has turned the USMLE from an option into a de-facto requirement. And the COMLEX won't go away anytime soon, because the people that administer it won't want to lose their jobs.

They are also the ones shouting that, "You don't need to take the USMLE. All allopathic programs recognize the COMLEX" to the herd. Sequentially, half of my school drank the kool-aid.

IMHO, when get rid of COMLEX, OMM shouldn't be even added to the USMLE for DO students. Each school should administer a final OMM exam like a clerkship shelf exam and call it that. OMM don't need to have its own place in national standardize exams.

What about grammar having its own place on a national exam for medical students? Haha...

But seriously, the two tests are staying for the long haul. The only way there could even be talk of a change would be a USMLE test and then an additional block of OMM questions for DO students. This additional block would cost an extra fee of course. The biggest hurdle to a single test is money.

You may say that we should all take an OMM exam at our schools before leaving, but that would not be "standardized." There are other differences within the COMLEX as well. I'm not a strong supporter of how the questions are written in the COMLEX, but it's not like the USMLE is some infallible test. They both have their pro's and con's.

I could see them merging the COMLEX into somthing like the USMLE-DO version. That way NBME can take the reigns on it just like ACGME took the reigns of AOA programs.

Ugh, my school has told us this several times - that ALL allopathic programs accept the COMLEX. It is frustrating to say the least. Its also ironic because at pre-matric events or in the newsletters etc. , when they are bragging about the programs students matched to, it is always the higher-profile ACGME programs; the same programs that students wouldn't have matched to had they just taken the COMLEX.
 
SDN really does tend to as we do put a spin on the whole COMLEX/USMLE thing. I mean clearly it's best to keep opportunities open and take both. But even at my school, which is top tier only about 50% take the USMLE and we still manage to have an impressive match list of programs and specialties.
Most DO applicants are probably going to be just dandy with a good comlex score because chances are they're going to want to go do a residency in PC and probably with a preference for being back with family or something than aiming for a high tier place with a major emphasis on a research career. Sure, you want to do IM at a university program, surgery, or want a career in academia, then do the USMLE. Don't want that? COMLEX probably will get you there too.

But that's my perspective. SDN is in somewhat an echo chamber big on the emphasis of borderline arbitrary boundaries such as mid tier or high tier and prestige.

It's a "top tier" DO school. It's a pseudo reference point that means little in the grand scheme of things. I only mentioned it to show that even what is supposed to be a bastion of higher DO student match is still barely getting over 50% of the class to take the USMLE.

Regardless, as I've said before. Most people at my school are probably going to care about getting a residency back home than in working towards high tier university programs. I'm not saying that everyone is like that at my school. But by in large most of the people I know are that way.

My school it's pretty heavily regionally focused as well, myself included. I'd rather stay close to home than end up in my top fields far from home, and a lot of my classmates are in the same boat.
 
Literally before last week I was going to try and get a cush job in the Midwest, but the longer and longer school goes on, the more I would rather move back to San Diego where I can be with my family, even if that means paying 4x as much for a home/gas/everything else you can think of. California is a damn ripoff!

I think it'll make them ACCEPT it, but it won't make them LIKE it. So sure, you'll be able to apply everywhere ACGME with a COMLEX only, but there's nothing to stop them from prioritizing applicants who have a USMLE score to offer, or setting the COMLEX bar ridiculously high.

Personally I think this is a reason for why we have particular outcomes including the distribution of board taking. If someone in a Midwestern DO school takes just the comlex chances are they'll be able to match most of the programs in their area.

Honestly I feel the same way. Not that I don't like Kansas City, because frankly I love it here. But unless people I care about move here, I'm going to end up going back home. And honestly, if that means that I'm not going to be in the best programs I can get into for my field, that's fine too.

Because in the past, people with USMLE scores could not apply to osteopathic programs. Moving forward, many osteopathic programs are covering to ACGME, non-osteopathic recognized programs that will receive Step scores in massively increased numbers, from US and international graduates. In the past, Step-only candidates did not exist in the AOA program pool, so direct comparison between candidates was simple. Now, to be as competitive as possible, you'll either have to blow the COMLEX to pieces or take both exams so they've got a consistent means of rating you against both pools due to the very different nature of the two exams.

Hence why it really should be the only board exam period as it does undoubtedly open up every door. But whether it's a requirement is debatable. Undoubtedly as time progresses its going to end up becoming more and more a requirement as most DO students will end up taking it by choice either way.
 
In order to keep the cash train they should just do USMLE with an OPP section and have the OPP section alone cost the same amount as it does now for the COMLEX. Win-win and I would be fine with that
