Oh, and BTW....

My family is buying me a new car for my graduation present!!!!
😱😀😎

I want a nice fuel efficient car. I am thinking about the Toyota Yaris. Any other suggestions?

I'm here now...many hours after you left probably.

Congrats to everyone on:

1) passing their boards :hardy: +

2) graduating from preclinicals to the wards :scared: +
3) graduating and becoming newly-minted doctors 😱 + :biglove:

4) getting job offers and getting ready to become attendings! + 😀

It's heartening that we're all part of this constant cycle in medicine...I can almost hear "The Circle of Life" playing from Lion King at this point.

I definitely heard the song as I was reading the post.

I also remember at this time thinking everyone was moving on but me. Now I'm one of them, and extra happy (well, as soon as boards are over)!

I was kind of disappointed today. I asked someone (who finished their third year..going on to 4th) what they wanted to do. Her reply was "I think EM. I really wanted to do surgery, it's really fun and exciting, but I feel like I'm going to be too old by the time I finish my commitments and I don't want the hours they have to do." I stopped paying attention after this point.

curiosity got the best of me. I heard on commercials last night they were going to do a special on pain management /undermanagement on the news. Here is what they told the public: Not too surprising here

😡 I know some have chronic pain, but they are now encouraging the drug seekers to find the 'right' dr so that their 'pain' will be managed with narcotics. uggh.

Wow, that's awesome. Oh, you'll be rollin' under the palm tree-lined avenues...😎

While some may not add much more efficiency on highways, hybrids make sense when you have the stop-and-go traffic of LA. Yes, more expensive upfront but there are tax credits.

For example, if 25 mpg is average for LA traffic, and you drive 200 miles per week and it's $4 a gallon gas, then getting a hybrid will:

If fuel efficiency increases to 35 mpg:
- you save $9 a wk, $475 a yr, and for 5 yrs save $2400

If fuel efficiency increases to 45 mpg:
- you save $14 a wk, $740 a yr, and for 5 yrs save $3700

If gas continues to get more expensive you save more. Just something to think about.

(Hey look, learning to do those xyz questions on the SAT/MCAT/etc did come in handy in the real world )

These are good websites that allow you to compare fuel efficiencies and look into tax credits:

Diesel Jetta.
Diesel Golf.
How big do you want/need? How many doors? Etc.

I like small cars, but would like to fit at least 3 others in the car if need be. Doors are negotiable. I really like the mini cooper.... 😍

I finally watched "I am Legend". Will Smith was hot but the science/medicine was way off. 👎

Congrats to the graduates (both med school and residency).

I just finished my last lecture of MS2.

I had that feeling about 2 weeks ago. It feels great, doesn't it?

I hate allergies. I felt miserable all night and didn't want to get up today, but I have no choice. ugh.

There is the slightly bigger Mini Cooper Clubman.

The new Ford Focus has the Microsoft Sync system.
Dodge gives you cheap gas for a few years as part of their rebate plan. Suzuki does too. You would have to do the math to see what ends up being cheaper (for your parents, anyway).
Saturn makes the new Astra (which is an older Opel/Vauxhall/Holden Astra, but Euro in every way). They make it in 3 and 5 door flavors.

Yeah. It's OK. I haven't attended class regularly since...before Thanksgiving, so I'm not terribly burnt out on class.

I am tired of our exams, which are total BS. So I'll get back to you after I take my last exam later today!

Good luck! :luck:

(and I hear you on the bs part)

I am reading through my HIPAA training for the first time. Why is it that it seems to sound like a bunch of lawyer jargon? Or is it just me being unfamiliar with the terminology? And is it repetitive with different ways to say the same thing, or is it my imagination (goes back to the terminology)?

It was terrible. I mean, I checked the lecture schedule when I got out to make sure I hadn't missed a class.

Try this one on for size:

Q: The fetal O2 dissociation curve is L-shifted implies:

A: More HgF than HbA
B: O2 should easily transfer to the fetus
C: O2 diffusion across tissue is improved
D: Fetus needs a greater O2 than the mother

(The poor grammar of the question [near verbatim] only adds to the problem)

My GUESS would have been A in this scenario. It seems to make the most sense of all of them. The way I understand it is that the mom will need more oxygen than the fetus because if the mom doesn't live, the fetus won't. O2 diffusion across tissue is not improved in a left shift. O2 should easily transfer to the fetus, but I'm guessing this is the distractor because there is some resistance to the O2 transfer in the placenta... Of course I could be more than wrong. I remember getting questions like this that I had no clue on (probably one quite similar). Do they ever throw questions out at your school?

Let's break this one down.

A: More HgF than HbA
While this is a true statement, it has nothing to do with the dissociation curve. There are many things that will shift the curve left, not just HgF, so to me, this answer assumed too much.

If they had asked something about Hgb electrophoresis, or the timing of when the thalassmias present, then this answer might make sense.

B: O2 should easily transfer to the fetus
HgF's increased affinity for O2 (not really increased, just less susceptible to the effect of BPG than HgA,) would make it easier to transfer O2 to the fetus. This one made the most sense to me, I chose this one. Even then, a better answer would be "There is a driving force for oxygen to be transferred from the mom to the fetus"

C: O2 diffusion across tissue is improved
This one depends on what "tissue" you're talking about. Mom->Fetus across placenta? Sure. Fetal blood->fetal tissues? No. Any left-shift of the curve makes it harder to drop off O2. That's why high altitude increases your BPG concentration.

D: Fetus needs a greater O2 than the mother
Greater O2 sat? Greater O2 demand? Greater O2 delivery? The incompleteness of the answer left much to be

Oh, and didn't I tell you? We never covered this in class! Yes, the faculty will remove questions, but that would mean ~30% of this exam would go.

Our cumulative had the most questions thrown out I have ever seen, and that was a total of around 6%...probably should have been more, but you know how some profs' questions never get thrown out. Your explanations make sense too...the question and choices were very unclear, and could have been conceived many different ways (as you can already tell). I hate those questions over things you think you have not covered (and the profs swear you had)...they always make me think more into the question than what is actually there.

When are your boards?

I got the impression that the profs just held onto the exam from previous years, regardless of little things like dropping lectures, changing curriculum. 🙄

I take my boards on 7/3. I can't wait until I can really see how good or bad of a student I am, and however I do, it is solely up to me. 👍

I understand that one. My boards are on 6/12. Coming way too fast!

Congrats!

I will be buying a Nissan Z. silver with black interior.

I also hate workign 3 overnights in a row. Sigh. 8 more shifts after tonight. (4 of them overnights).

ah, so THIS is why it's dead in here!

:meanie: I've actually been visiting here too frequently, but decided to refrain from posting, which has helped with the time factor. Now if I could just refrain from checking this site as frequently...too bad a lot of what I do is on the computer and on the internet...

I decided a few weeks ago to study outside to get away from the computer and just study the books. Definitely a bad idea when I forgot to put on sunscreen and I forgot how bad the allergies got when everything started pollinating. Definitely still paying for it.

Actually concentrated for the longest time ever today. Like straight concentration for 4 hrs. And for me, that's a miracle. I'm going to try to do it again tomorrow...we'll see if it happens. Wait, it WILL happen. That's what I have to say for it to happen.

just got a call from the housing office (they called me doctor when they started talking to me... very bizzaroooo) anyway, they said they can have my apartment ready by june 2!!! yeah!!!! that made my day yesterday

yeah, it's weird to be called doctor. I have to keep fighting the urge to yell, 'stop patronizing me! We both know I'm not a doctor yet!' 😡...but I guess I am now :scared:

ugh, i am up with bad gastritis

the nausea, the burping, the epigastric pain....

haven't had mmuch caffeine and no NSAIDS, so I figure it must be stress

I am somatisizing my need to move cross country, get a new place, and buy stuff, all on a small amount of borrowed money

I think I will be eating $1 mcdonald's sandwiches during saem, but it will be worth it to be on the research committee 🙂

and shall I ever receive my schedule so I can start furiously studying?

wowsa, look at that whinefest disguised as post-padding! Sorry, all

I think I'll pla solitaire on my pda til I fall asleep. Post-padding takes too much thought

speaking of thouht, I leave you with this one: why does salivation proceed vomiting? I figure it is just a vagal/cholinergic thing, but why? Is it because salivation will help protect th oral mucosa from stomach acids? Anyway, here's to not throing up tonight.. :barf:

We were actually taught the mechanics behind it twice within the past year. When I'm less tired I'll try to remember if this is it or if it's something different. I know that part of it had to do with preparing for the acid. And then there was something with the lovely vomit center of the brain.

They'll let you borrow more. You just have to ask. Not saying you should borrow more than you need, but you can always get enough.
You can get more than you need as well, but why?

Did you buy a car yet?
