Because I took my 3 hour nap, I am paying for it and having to study intensely and pretty late. Otherwise I would stay and chat.

Sorry. I have been a slacker all week. Exams on Mondays kill me because I can't sleep the night before, then I pay for it until I get some sleep either during the week or the weekend after.
Any ideas on how to formulate a well-organized soap note without memorizing someone else's?

I always do:
overnight events if any (no one likes to be surprised halfway through when you say they had to undergo 3 rounds of cpr)
Subjective - how the patient feels
objective - vitals, I/O, physical exam changes, pertinent labs
assessment - summary sentence with "now improving with" or "complicated by"
plan - by problem list for medicine, including prophylaxis and dispo
write it down the first few times before presenting, then it will become second nature
Hmmmm...I have that part. Just the details they said were really not in the order they wanted them. Most of them were there, just not in the order that made sense to them. Probably didn't help when they were reviewing the video (these were mock pt interviews) and when I sat down to summarize things, I started asking the questions I forgot earlier. I guess they don't like the randomness and ADD when you interview and write up the soap note.![]()

Ya. like normocephalic, atraumatic, PERRLA, and stuff like that.
oh, ok - that should be in the Bates order (the regular physical exam order in the H&P)

I guess I just like to be regularly irregular when it comes to that. Not a good thing. Making some cappuccino...mmmm...need some warmth right now. And the caffeine won't hurt either. Thanks!
