Hello,

So you cannot access any part of the uc davis secondary app after you've submitted it, but now i want a copy of the instructions.

for anyone who has not submitted their uc davis application, can you go to the first page of the app where it says the instructions (and that handy box that checks to make sure you've read it carefully) and copy and paste the instructions for me?

i would be very very thankful. 🙂

We are excited to receive your application for admission to the University of California, School of Medicine, Davis. Congratulations on making it through our initial screening process. Further evaluation of your credentials will begin upon receipt of all supplemental materials required to complete your file. The enclosed packet of materials and our request for supplementary information is asked only from those applicants given further consideration for admissions. Given the thousands of applications we receive each year we found it necessary to set some guidelines.

Please be assured that at UC Davis each and every application is reviewed individually in an attempt to secure a well rounded, diverse and outstanding class. You are not merely a number and we will carefully review your complete packet to see if you might fit with our philosophy and approach to medical education.

We ask that you please read the following information to insure that your application will be processed in a timely fashion. All materials requested by the Admissions Committee must be received on or before 30 days from the date a secondary application was requested. After this date, your application may be withdrawn from further consideration without the requested secondary materials. Note: We will allow for a delay in receipt of the letters of recommendation after the given deadline.

NOTE: There are two (2) parts of the secondary materials. You must complete and submit sections 1 and 2 in order to complete the entire secondary application for UC Davis School of Medicine. A section is related to the new Rural-PRIME Program.

Failure to follow these instructions will lead to a delay in the processing of your application.

  • The Admissions Committee asks that you do not send videocassettes, audiocassettes, CD ROM disks, books, magazine or journal articles, research proposals, or transcripts unless requested.
  • Change of address, telephone number and e-mail address can be made through the internet. The link is located on the admission status cover page http://www.ucdmc.ucdavis.edu/ome/admissions. We will utilize e-mail to communicate with applicants as much as possible. If available, it is important that you provide the Office of Admissions with an e-mail address where you may be reached. Check your e-mail often.
  • Letters of Recommendation: Three to five letters of recommendation will be accepted. Letters from your college Pre-medical Advisory Committee, a college letter service or electronic letter service are accepted. You may disregard the waiver forms if a committee or letter service is available. If your college does not have such a committee, appropriate letter writers include science/non-science instructors, employers, community service agency directors/coordinators, and faculty members supervising undergraduate/graduate research experience. Please indicate in this application which letters we can expect.
  • You may waive your access to letters of recommendation under the Family Educational Rights and Privacy Act of 1977, and other laws. Please refer to the Waiver of Access to Confidential Letters of Evaluation Form (the link to this file will appear after you submit this application). If you have waived access to your letters through your school's Pre-medical Advisory Committee, it is not necessary to complete the waiver.
  • Application Fee: A non-refundable application fee of $60.00 is required. The check or money order should be made payable to the UC Regents. If you have been granted a fee waiver from AMCAS , our application fee will be automatically waived. Please submit a photocopy of the AMCAS Fee Waiver approval form. Place both your SSN and AMCAS ID number on the check or money order.
  • Supplemental Personal Comments: Please answer the questions on the next pages of this form. Confine your comments to the space given for each question. You may cut and paste your responses onto the form.
  • Supplemental Personal Information Questionnaire: You may be asked to present substantiating documentation, or if granted an interview, you may be asked questions about the information. Your responses on this form will be reviewed and compared to the information on the AMCAS application. Unfortunately, we are unable to accept any attachment to the questionnaire.
  • Photograph: A signed wallet-sized (2"x2") photograph is optional. We use this photo to help interviewers identify you and to help them recall you at a later time. You may enclose the photo along with your check or money order. Please do not staple the photo to the check.
  • MD/Ph.D. Dual Degree Program: Please submit an essay expressing your interest in our Physician Science Training Program (PSTP) (see page 5 of this form). Information about this program is found on the Internet at .
  • Rural-PRIME Program: With a founding mission that emphasizes community services, UC Davis School of Medicine recently launched a new program to help increase health-care access for underserved populations living in rural areas. This is a combined, five-year M.D. and master’s degree program (M.P.H. or M.S., Informatics). It is specifically geared toward students from rural backgrounds who have a strong desire to make a difference in communities like the ones they were raised in.
  • Permanent Residency Verification: We require a copy of your Green Card be submitted along with check or money order if you have been granted Permanent Residency in the United States
  • Disadvantaged and Special Consideration Essay: Please submit an essay describing your life experiences and circumstances concerning the nature of this designation (see page 6 of this form).
NOTE: It is each applicant's responsibility to insure that ALL requested secondary materials have been completed and returned to our office. Incomplete applications may be automatically withdrawn from further consideration.

When entering text, do not use formatting codes such as bold, underline or italics. Before you cut and paste your text from a word processor, make sure you have first saved the file in plain text format. Double quotes need to be added after the text is inserted into the form.
