Since there seems to be some confusion/lack of knowledge, here is how the hold process works at Mayo:
If you are on the hold list, you are in a pool relatively equal to each other. There is a ranking system, but it's not always followed.
Mayo sends out decisions biweekly (I will confirm this tomorrow), and at every decision meeting, they go over newly ranked applicants in addition to applicants on hold. At every meeting, we will be looked at and considered for any acceptance.
Typically, about half of the class comes off the waitlist. This ends up being ~20 people at different times, though I am not sure if it includes people who are accepted off the hold list and before the end of interviews. I assume it does. This means that, in effect, not many people come off the hold list, but some most assuredly do.
