Can I make it into a top 20 school?

3.75 GPA 3.9 sGPA 517 MCAT
Already graduated so my GPA's is not going to change.

I have a first author publication and 2 co-author publications.
If I do not get rejected I should have one more first author publication and two other publications.

ECs

Division I Athlete

A lot of volunteer work. Some examples include working extensively with kids with special needs, volunteering at a nursing home and working as a case manager. However, I do not have the hours recorded for these things. I just did them, because I wanted to and I never really kept track. Do I have to confirm my hours with someone? if so what should I do? I can definitely confirm some of the hours, but I never managed to track most of it.

I worked in a lab for 2.5 years in school did a thesis and presented it at a conference.

I currently work in a hospital doing clinical research. I have a lot of patient interaction, I do EEG's, ECG phlebotomy etc. and help with drug trials.

Various clubs

I also I headed one club for two years, but it got shut down due to the sponsor getting fired. It is a long story.

I also have some interesting stories about fundraising money for research and helping to write a grant to research therapy for kids with special needs. I do not know where i could put that in my application.

EMT-B license got hired worked for like 2 days and then got a job where I could actually pay my rent haha.

Shadowed for well over 70 hours with different doctors I have seen a gastroenterologist, a hand specialist, a neurosurgeon, and neurologists. I have been in the operating room multiple times.

Can I possibly get into a top 20 school, or is that out of reach? Is that a pipe dream for me? What can I do to improve my chances?

The stats plus the D1 athlete makes me think you definitely can get into a top 20. You're shadowing and leadership is good. You should go through and make a list of all the hours for volunteering and split them up into clinical and non clinical categories though.

Goro and others on here will probably give you a good idea of what specific schools you're good for though.
 
Maybe a dumb question but does it count as clinical volunteering if i didn’t volunteer under a docto, but rather in a clinic with PT, ot, speech or a nurse

Of course! Only thing you need to make sure is with a doctor is your shadowing.

Edit: as long as you interacted with patients

Yes, definitely, granted you write and interview well. If you can afford to apply to all top 20 go for it!
