Podiatry never ceases to surprise me, which is why it always provides us with an abundance of meme content.
Lol this guy came to my school to give a talk about ABFAS boards when I was a student. There was a question for him regarding clarification when third year residents take their in-training exam and what it entails for board qualification.....He did not have a straight answer.........

Imagine baiting grown adult doctors with a Starbucks gift card and not only proposing that, but not following up on it.

I've noticed a lot of wound care docs (DPMs and MDs) will tell pts to "stay off of it," which doesn't really mean anything. And of course when the patient doesn't "stay off of it," they can document that the patient was noncompliant with the medical advice that was meaningless to begin with.

This is what total contact casts are for.

They work great. The peace of mind with daily dressing changes is easier though. With TCCs you don’t know if stuff is getting soiled or an infection is brewing until it gets bad.

But to be fair, noncompliance with medicine or self neglect is likely what caused the ulcer in the first place through uncontrolled diabetes or drug use, what have you. The patient has to want to change and TCC is really one of the few options outside of surgery where you can force a change on them

I inherited a wound clinic pt and the doc over there (MD trained in PMR) made her feel like dirt because she wouldn't "stay off of it." Ditto with my residency director, he used to tell pts to stay off of it and PT would page me asking what that translates to for a wb status.

Honestly it’s pretty straightforward. If someone tells me to stay off my foot, I’ll find a way to not put weight on it. It’s a CYA thing. But also real advice that a standard patient would follow.

Then again, I also didn’t get myself a hole in the bottom of my foot and not notice it. We don’t have “standard patients”.
