"Other Names" is usually used if you have had...well...other names. 😉 People change names for a variety of reasons (e.g., maiden name to married name). Some schools will ask both your current and "other" names for, say, the purposes of clarifying application materials where your name may be different in some things (e.g., financial information). For example, the name on your transcripts may be different because you got married at some point during or after graduate studies or because you changed it, I don't know, to be cool, perhaps. 😀 The name on your bank checks may be different because your current/new name has not yet been incorporated onto your bank accounts - another example. This will help schools "put 2 and 2 together" when organizing application materials. It has no bearing on admissions, it is for the sake of efficiency.
If you do not have "other names," then I would leave it blank. I left mine blank.
