2022 NHSC Scholarship Application

This forum made possible through the generous support of SDN members, donors, and sponsors. Thank you.
Anyone know how to fill out the W-4 form? It talks about employer but I no longer work as I already started dental school. Who do I put down as my employer now?

Fingers crossed and prayers up for those who haven't received the email yet!

The NHSC is your employer for this situation. Leave that part blank for them to fill out.

It’s not binding, this finalist notification is just saying we are still interested. In September, after they hear back from all rounds of finalists, they will send us an official contract to sign and that will be binding.
Not true. Part of the finalist paperwork is you signing the official contract. The next step in the whole process is for the NHSC rep to countersign it making it binding/official that you got the scholarship. There may be a small window for you to back out after this, but this is pretty much it.

where is the contract? i’m only seeing the W-4 and the letter of good standing.

So for the W-4, we only fill out Step 1 if we file separately/not married/have no job (in dental school currently)? And leave the rest blank??

The last step of the finalist paperwork is entering your password or something and that electronically signs the contract. After you have everything submitted, there is an option to download the contract. That is also the way you can look for the counter signature when it happens. A lot of people will periodically check for a counter signature as a sign that things are moving forward before the final notification of the award.

EDIT: I just looked at a W4. You should fill out step 1-5. This is so they know how much federal taxes to withhold from your stipend. If you don’t have a job/spouse/want additional withholdings, fill that out/leave it blank accordingly. They will fill out the bottom line about employer.

Great, thank you. Do I need to sign the “employee’s signature” on step 5, even though I am not employed?

I will leave step 2, 3, 4, and Employees only section of 5 blank.

Yes. Because the stipend is taxable, the IRS has to consider the NHSC as your employer and so you are the employee. You should sign it under employee signature.

Does anyone have the PDF for this???

Yay!! Congrats to everyone who received the finalist email today!!!

What are you all putting for W4 Form, Section 4b - Line 4 where it asks "Enter an estimate of your student loan interest....etc, etc"? I do have student loans from college/dental school so far, but are people claiming any deductions here or should I leave it blank?

Hi everyone! I believe I requested for 4 years for scholarship funding. If I receive funding for all 4 years, is it possible to terminate the contract after 2 or 3 years (and still work the 2-3 years at a heath center)?

I doubt it would be beneficial for you to claim it on your taxes while in school. I would just leave it blank if that is the only thing you are considering.

There is a way to “refuse” funding during any semester you have the scholarship, but it does not reduce your commitment as far as I am aware. I would HIGHLY recommend not planning on terminating early or trying to change anything in the contract after you sign it. If you want to only do 2-3 years, only accept 2-3 years right now at the finalist stage. You can always apply for a continuation later if you decide you want to do the entire 4 years. It is a hassle, but not nearly the headache/heartache you would cause by trying to cut the contract short.

Is anyone else here on team #noresponse? Feeling nervous about the next few waves of finalists... I got the credit check back in June, but haven't heard back from them since. Hopefully the credit check is a good sign and we'll be getting our emails soon!

oh they have another round of finalists? is it after some finalists reject it? or are they still reviewing?

Yes, from previous years it looks like there are multiple waves of finalists! So keep a lookout for an email or check your portal weekly.

Of course with any good news my anxiety likes to ruin things a little - does anyone know why that 0.01% of finalists for any reason do not get the scholarship?

After submitting forms, the site took you to a new page where there was a preview of the contract.
There was a section that said "Applicant Name", date, etc. and then a space below to electronically sign the contract.

I was able to electronically sign the contract just fine, but there wasn't a method to write "applicant name"/date/etc nor upload a filled out version of this contract. So I signed it and submitted it as is, and it went through.

Is it safe to assume that's just how the completion of the contract is supposed to be (without filling out the "applicant name"/date portion of it?)

Yes. In your portal, there should be the option of downloading the contract. When you download it, on the very last page, there should be text that says “electronically signed by:” and then have your name, your role (applicant) and then date/ time.
Below this is also where it will tell you when it is countersigned. It will say “electronically signed by:” and then have the name, role (OFAM approver or something like that) and then date/ time of the person at NHSC/HRSA. Lately the name has been Jamie King.

Thanks @cooliyak for spending the time tonight to answer all the new finalists' questions! Really appreciate you being the online, after-hours liaison 😊

In terms of funding, theres a stipend + "ORC". Does the ORC differ for every applicant and school-dependent? About how much is this ORC payment, and is it also monthly or just a one time payment?

No problem!

ORC differs per school per applicant. It is yearly (every Oct) and calculated based off of the cost of attendance that the school sends the NHSC. It is supposedly to cover all of the required fees above and beyond just tuition (such as books, etc). The first one is usually larger because they just throw in an extra couple thousand dollars for everyone to buy a new laptop.

They should be scheduling a webinar for all of the new applicants to go into more details about it. A couple weeks after you get your ORC, they usually email a generalized list of how they determined your ORC amount.

Anyone know when counter signatures came out in previous years?

Anyone know if they wait to change your "Under Review" status to denied before Sept 30? is it like dental school where you don't have a hard "no" until the cycle closes?

Also- do applicants ever get credit checks and *not* get the award?

I'm a finalist. What if you switch schools (got accepted to a diffrent school after submitting application). I contacted NHSC to switch schools but was told I may be DQed, and asked to be escalated to NHSC representative. Apparently I'll get an email back.

So should I fill out EVF for the school I marticulated at ? Classes started July 25th.

Panicking here. Have exams as well.

*** Does EVF need to be filled out by financial aid or Admissions office or another office? Registrar ?
I’m in my second year of dental school so I had my dental school admin office sign it. I’m not sure who you would need. I guess whatever school you’re at for the fall term needs to complete it. That might be a question for NHSC. I hope they don’t disqualify you over the school change. Fingers crossed for you
