As a service to the SDN students, I will be answering any questions that you might have about the pathology material. Please don't ask dumb questions,

In terms of the questions that you may want to ask me, limit it to Step 1 material! Not about your grandma with this disease, or that. That's not what we are here for. I am here for Part One material only. I have heard every single type of question there is to ask, so I anticipate them. So that's it.

Shoot!

Do you also take phone calls? I mean, while you're at it, might as well come to my house to teach, eh? I'll pay you too 😀

Right...the chair of Path at OSU, STEP 1 reviewer, and the topic of several threads on these boards is willing to take a bunch of questions from us 😉

I will only take questions on SDN because of my grandkids. I'm the absolute best papi in the world. I have three of them hanging on me at once right now. My 5 year old is Superman, my 3 year old is Batman, and I'm Power Ranger! Let's get back to questions about the boards.

Here's an easy one: serum sodium of 119...diagnosis?

The lowest sodium you will ever see is inappropriate ADH. I am going to give you a pearl for the SDNers, on a board exam Step 1, serum sodium below 120 is ALWAYS inappropriate ADH. You have to expand your knowledge on SIADH. As an intern in redding, pennsylvania I saw a woman with sodium in the low 80's, and she didn't have oat cell. She was a type II diabetic, and was on a sulfonylurea.

Dr. Goljan is absolutely right.

The first-generation sulfonylureas, except for tolbutamide, have largely fallen out of favour due to significant adverse effects including protracted and refractory hypoglycaemia and the syndrome of inappropriate anti-diuretic hormone (ADH) secretion (SIADH).9 The second-generation sulfonylureas (e.g. glibenclamide, glipizide, gliclazide) became available since the 1980s, and these agents generally have more acceptable side-effects profiles.

Sulfonylureas

Papi! I've got all of your albums. They contain so many wonderful clinical pearls! I've listened to them all and I have to say that I would give them a pretty good graaaade.

First year and you already have goljan audio 😱 .

Considering that Dr Goljan is lecturing in Tulsa on the 24th & 25th, but this poster's IP is from a significantly distant state (plus the fact that Dr. Goljan's middle initial isn't "J"), I feel safe in saying this is an imposter.

Closing.
