After decades of talking trash about how boring soccer is, my instagram feed convinced me that it was worth my time to watch. I've now watched two world cup games that weren't the US............ Curacao vs. Ecuador and Belgium vs. Iran.

So - is there anything you aren't telling us. Like - was there a $120K base and then it was RVUs or was it just straight RVUs. Were they paying more for self-pay crap or pushing laser or ancillaries that added something more? Was there a distribution from a PE owns surgery center or some sort of stupid quality bonus? Cause otherwise that's just straight you-are-screwed-poverty-going-no-where wages. Like - not kidding - less than 30% of collections type money.

I have an insurance that pays $323 for a 11750. 30% is $90 something. 1.54 wRVU is $30?
I have a few insurances that pay about $90 for a 20550. So like $30 for you on 30%. But 0.7x$20 in the above.
One of my insurances pays incentive money for surgery at an ASC. So like ..$1400 I think for a lapidus. So like $400ish for you at 30% and 9wRVU at $20 is sub $200?
I have plenty of bad insurances too, but it just seems like they didn't do any "blending" of plans. They just went down the line and picked the lowest value.

Use an insurance broker. They shop around with a bunch of companies for you. PICA will be the highest quote you get. Literally any other company who provides tail coverage to Podiatrists (not all do), will be cheaper than PICA.

There's some market stupidity coming out of some otherwise smart people here, so here's a free economics lesson from a small business owner:
Employers don't set wages, the market sets wages. You can't shame a company into paying more than $20/rvu. And they're not going to pay more out of the kindness of their heart either. This isn't exploitation, it's supply and demand. Upperline is the symptom, not the disease.
A lot of you are pulling in massive salaries and have phat retirement accounts. When you invest, are you picking companies that inflate their payrolls for no reason or do you pick the ones that maximize profits for the best ROI? Would you take a chunk of your wealth to start a podiatry practice and hire a new grad at $460k to see patients there?

They may be a symptom, but it still just seems cartoonishly evil.
In the spirit of your symptom verse disease though - private equity does depend on entrapment/exploitation of the insurance market. Get rid of non-competes and create a world where there's only Medicare and BCBS and private equity doesn't exist. Treat an associate poorly - gone, across the street. Medicare let's everyone in and pays retroactively. And BCBS credentialed me in 15 days and told me "we pay everyone the same". Anyone who wants their own office could make it work in a world like that. No Aetna, Humana, United mostly means no denials and administrative work. Its just comedic how bad the current system is. I sometimes think we do have Medicare for all - just in the worst possible way. The doctor gets paid less than Medicare. The patient pays $15K in premiums and has a $15K deductible and the doctor has to collect it all up front. And the insurance company denies anything they can and jumps us through non-stop hoops.

On the contrary, there are plenty of docs who pay their staff above market rates because they realize they get what they give. Just because a large employer can leverage their scale to pay lower wages doesn’t make it a genius move.
