Hi everyone whose appearing for the nbde part 1 for jan or feb (or anytime early next year).. Im starting this thread so that we can all motivate each other, and keep each other updated on our progress. so that we dont get lazy, but rather stick to schedule and stay focussed.

**Please dont ask for books, or material or study partners in this thread.

Lets keep it solely for
1. Motivating each other
2. Sharing any mistakes or tips to study better
3. Asking questions during preparation
4. Anything you wish to share with others while you are studying
5. Review or questions regarding books, decks, tests that you are using during this study period

Thanks, and i wish good luck to my fellow test takers 🙂

If a patient complians of TMJ sharp pain on mandibular movements, which intracapsular structure is the most likely to be involved ?
a)anterior portion of the articular disc
b)stlyomandibualr ligamnet
c)retrodiscal tissue
d)articular cartilage
e)articular capsule

I think the answer is C) Retrodiscal tissue.

The answer C was what I felt was the right answer. I'm still not 100% sure, but its what i would have written on the exam. Here's something I found online right now to check if there was an answer online:

(from wikipedia: TMJ)
The most common disorder of the TMJ is disc displacement. In essence, this is when the articular disc, attached anteriorly to the superior head of the lateral pteygoid muscle and posteriorly to the retrodiscal tissue, moves out from between the condyle and the fossa, so that the mandible and temporal bone contact is made on something other than the articular disc. This, as explained above, is usually very painful, because unlike these adjacent tissues, the central portion of the disc contains no sensory innervation.
In most instances of disorder, the disc is displaced anteriorly upon translation, or the anterior and inferior sliding motion of the condyle forward within the fossa and down the articular eminence. On opening, a "pop" or "click" can sometimes be heard and usually felt also, indicating the condyle is moving back onto the disk, known as "reducing the joint" (disc displacement with reduction). Upon closing, the condyle will slide off the back of the disc, hence another "click" or "pop" at which point the condyle is posterior to the disc. Upon clenching, the condyle compresses the bilaminar area, and the nerves, arteries and veins against the temporal fossa, causing pain and inflammation.

The bilaminar area includes the retrodiscal tissue (i believe) + it is heavily innervated. So thats my answer. (was debating between that, articular capsule and ant. portion of disc due to it being the site of l.pterygoid insertion)

:help:
Can anyone answer this?

a)anterior portion of the articular disc
b)stlyomandibualr ligamnet
c)retrodiscal tissue
d)articular cartilage
e)articular capsule

Answer is E,
Posterior retro discal tissue IS highly innervated, but it's main role is nutritional. Capsular and collateral discal ligaments are involved in joint position, movement and pain. Hope i'm right. my exam is a week away!

e)articular capsule

answer is c) retrodiscal tissue

explanation: 3 periarticular tissues containing nociceptors: 1) discal ligament,
2) capsular ligament 3) retrodiscal tissue . When these ligaments are elongated or if retrodiscal tissue is compressed , the nociceptors send out signals and pan is percieved. The person cannot differentiate among these three structures so any one of them when stimulated radiates pain.

So as per this we can deduce that c) retodiscal tissue and e) articular capsule are both responsible for the sensation of pain but the question states that which INTRACAPSULAR STRUCTURE is responsible for this pain, which makes it retrodiscal tissue.hence the logical choice seems C) RETRODISCAL TISSUE

plz can someone explain me where nicotinic receptor and muscuranic receptors present in synapse and how does they action?

Both nicotinic and muscarinic receptors are types of cholinergic receptors. Nicotinic receptors are present on dendrites and cell bodies of both sympathetic and parasympathetic postganglionic neurons.Muscarinic receptors are present on all effectors(muscles and gland) innervated by parasympathetic postganglionic axons. most sweat glands and smooth muscle in certain blood vessels which recieve their innervation from cholinergic postganglionic sympathetic fibers also display muscarinic receptors.activation of nicotinic receptors lead to depolarization and thus excitation of postsynaptic cell, be it a postganglionic neuron or a visceral effector. activation of muscarinic receptors sometimes cause depolarization(excitation) and sometimes hyperolarization(inhibition) depending on which cell bears the muscarinic receptor. ex: inhibition is seen in smooth muscle sphincters in git(relaxes) but in smooth muscle fibers of circular muscles of the iris of the eye to cause contraction, this is where excitation is seen.

thank you guys for the answers..
amalgamted80 could you please tell me the reference
thank you so much 🙂

thanks amalgmated that was very confusing
when do u hav exam?

sorry saara , i tried:-( havent taken exam dates yet , hopefully sometime in may/early june depending on how much i get accomplished in the next few weeks. goodluck on your exams and hopefully someone else can explain this to you better than me🙂

Hi all,
I have been reading these posts and they helped me a lot with exams. Thanks to everyone.
Gave my exam last week and thought I would share my experience.
So, to begin with... the exam was EASY.About 60-65% questions were basic level direct questions which could be answered easily . Very few questions were indirect and confusing and few of the questions were direct, but answer choices were confusing.
There is no shortcut to ace this exam. I did not find NBDE first aid very useful as they have so many errors and the book is full of buzz words. Its of no use. Utilize time understanding the topic as few questions need you to think.
Not many repetitions at all. The ones that were ,were all basic to mid basic level. That was disappointing.
Questions from tough topics like neuroanatomy and neurophisio were a joke. I could have answered them with my memory from BDS!Just 2-3 questions.
Had vague questions from liver, pancreas and lower limb.
Still,I would say it was not bad at all.Kaplan Q bank helped me a bit.
Materials I followed...Decks,Kaplan review notes and Q bank( Especially micro and physio),USMLE for some topics, ASDA papers (all the years),Wiki.
When you are just 2-3 wks shy of giving your exam, don'y waste time looking for remembered questions or looking for answers for weird questions. Total waste of time. There are not many repetitions at all .
I have noticed, questions from topics vary on each exam and can't really predict a pattern.
Stay calm and good luck to all.

Hello Dream.

How many questions of lower limb were on the exam, cause since everybody says is not important i skipped that part while studying anatomy, were they difficult? Do you think is worth going back and revise that??

Thanks in advance 🙂

capillary diameter is directly influenced by
1. sympathetic n.impulse
2. parasympathetic n.impulse
3. byproduct of metabolism

given ans ic 3.byproduct of metabolism
why?

hey bunty, my intial answer was by-product of metabolism as well. the reason? parasympathetic and sympathetic nerves affect the vasculature's smooth muscle which causes constriction or dilation of the vessel : mainly Arterioles.

capillaries have no smooth muscle! they are just a single lining of endothelium.

Thank you for that explanation... Lesson learnt - read the question carefully!

Good Luck to all

Capillary "diameter" is usually affected in inflammation or hemostasis. Adenosine is a product of metabolism usually responsible for this, which is a vasodilator.

Also, the other two choices, as stated above by Amreki, are responsible for controlling the smooth muscle of vasculature which capillaries lack.

Am i right? (referenced from kaplan if my memory serves me right)

Hey haam!! when is your result getting out brother??? my exam didnt go that well .. do temme when your result gets out , mine would be really close then...

Congrats!👍 bhavi4all...that isnt a bad score at all. I'm sure, u'll be able to get in some where, just apply to as many schools as possible...Good luck and keep us posted...would love to hear all about it..
And if u can share ur study strategy and the materials that u used and referred, that would be great..

hey...materials i used are nothing extraordinary..i jus did

kaplan review books
decks 09-10
first aid
and lil bit asda..

thats it... i didnt use Qbook , or mosby or that DA book which everyone says to use.cant remember its name.
and about strategy..its like everyone says , stick to the basics..keep on reading again.first reading i gave was book wise..like i finished review books..then i started decks..and then first aid...and so on...on consecutive readings i did subjectwise..like start one subject and do it from every book..and so on..
thats it.. any other detail , any one can ask..

hi ...i would like to know that if i should wait for the latest ecks which will be out till end of march 2011 , is it worth waiting or i should order the last years decks .....

hi i am also waiting for the new edition f dental dencks planning to give part 1 by the end ofmay......if you have any info please share tx

Well it totally depends upon you.How much time you have , and when you are planning to take the test. You can start with kaplan review books , and wait till the new decks come out.Or you can just buy the old ones and start.I dont think new decks are gonna be that different. But thats just a lame guess.People are scoring good with 06-07 decks till date.So totally your decision.

How did u feel about patho? do u think, decks and kaplan covered everything that was asked in the test? and also, did u concentrate on every single topic in kaplan for patho, or did u eliminate some based on whether u personally found it important or not?
I personally don't like microbiology in kaplan. what do u think?

Well patho is my favourite subject...i mean was my favourite..and i do think its enough in review books and decks... i didnt eliminate any topic as it is .. but there are few topics in review books, which seem that its a lot of info , which we dont need...but i still did it.. neva skipped it..well the rule is You read alot, you retain less...so always read more...
and you know , micro and patho saved me..the score i have is all coz of patho/micro..i scored highest in them..and anatomy ruined it for me....
